60例风湿病患者尿蛋白的变化及影响因素相关性分析
Correlation Analysis of the Changes in Urine Protein for 60 Cases of Rheumatism
【摘要】 目的观察风湿病患者尿蛋白的变化及其与实验室指标的相关性,探讨风湿性疾病对肾脏的损伤程度。方法检测60例风湿病患者尿液中尿蛋白的表达,同时检测血常规、尿常规及生化指标,随机选取20例正常健康者,检测尿液中尿蛋白的表达并与60例风湿病患者尿液中尿蛋白表达进行比较。结果风湿病患者尿液中尿蛋白指标尿免疫球蛋白G(尿IgG)、α1-微球蛋白(α1-MG)、β2-微球蛋白(β2-MG)、尿N-乙酰β-D氨基葡萄糖苷酶定量(NAG)、视黄醇结合蛋白(RBP)表达明显升高,尿蛋白异常率最高的为尿NAG(58.3%),其次为尿α1-MG(40.0%);相关分析显示:尿IgG、微白蛋白(mAlb)、微白蛋白/尿肌酐(MAl/C)、NAG酶与尿红细胞呈正相关(P<0.05或P<0.01),尿IgG、α1-MG、NAG酶、RBP与疾病活动度呈正相关(P<0.05)。结论风湿病患者存在肾实质的改变,主要表现为肾小管的损伤,同时伴有肾小球滤过的受损,肾脏的损害程度随着疾病的活动而加重。
【Abstract】 Objective To observe the changes of urine protein and the correlation with the laboratory indicators for the patients with rheumatism and explore the damage degree of the kidney associated with rheumatic disease.Methods The urine protein expressions were detected for 60 cases of rheumatism.Meanwhile,blood routine,urine routine and biochemical indicators were detected.20 healthy persons were selected randomly and the urine protein expressions of them were detected and compared with those of the patients with rheumatism.Results The expressions of urine IgG,α1-MG,β2-MG,UNAG and RBP were increased obviously.The urine protein abnormality rate of UNAG(58.3%)was the highest and that of urine α1-MG(40%)was of the second top.The correlation analysis showed:urine IgG,mAlb,Mal/C and NAG were positively correlated with urine erythrocytes(P<0.01 or P<0.05).The urine IgG,α1-MG,NAG and RBP were positively correlated with the disease activity.Conclusion The patients with rheumatism have the substantial changes of the kidney,which is manifested mainly as the damage of renal tubule with the damage of glomerular filtration involved.The kidney damage is aggravated with the disease activity.
