与肝癌肝移植术后复发相关的肿瘤组织标记物筛选

【摘要】 目的在肝肿瘤细胞中筛选与肝癌肝移植术后复发相关的差异蛋白,为预测患者预后寻找更灵敏、特异的生物标记物。方法 19例符合"上海标准"的肝癌肝移植患者纳入本项研究,6例术后出现肿瘤复发和转移(复发组),其余13例患者均无瘤存活(无瘤生存组)。利用表面加强激光解吸电离-飞行时间质谱技术(surfaceenhanced laser desorption/ionization-time of flight-mass spectroscopy,SELDI-TOF-MS)建立肝肿瘤细胞蛋白质指纹图谱,生物信息软件(Biomarker Wizard)比较两组之间的蛋白质差异。结果利用弱阳离子交换(weakcation exchange,WCX2)蛋白质芯片在质核比(m/z)2 000~30 000范围内共检测出163个蛋白峰。在建立的蛋白指纹图谱中,复发和无瘤生存组相比较,6个蛋白差异有统计学意义(P<0.05)。复发组中上调蛋白4个,m/z分别为212 9、220 3、295 0和306 2;下调2个,m/z分别为370 8和118 56。结论由SELDI-TOF筛选出的肿瘤细胞差异蛋白对判断肝癌肝移植患者预后可能有重要意义,差异蛋白可能与微血管癌栓形成有关。

【Abstract】 Objective To screen differential proteins associated with recurrence from tumor tissue in patients undergoing liver transplantation for hepatocellular carcinoma(HCC),and to supply the research basis for searching more sensitive and specific biomarkers involved in predicting prognosis. Methods A cohort of tumor samples were obtained from 19 patients meeting the "Shanghai criteria".Among them,6 samples were selected from recurrence group and 13 from disease free survival(DFS) group.The protein spectra of tumor cells were created by surface enhanced laser desorption/ionization-time of flight-mass spectroscopy(SELDI-TOF-MS),and the intensity values for each peak were analyzed by Biomarker Wizard Software to screen recurrence-associated biomarkers. Results According to protein fingerprints determined by weak cation exchange(WCX2) chip measurement,a total of 163 protein peaks were identified at the mass to charge(m/z) value ranging from 200 0 to 300 00.Six proteins were significantly associated with recurrence when compared with controls(P<0.05).Four proteins were up-regulated with the m/z value of 2 129,2 203,2 950 and 3 062,and two proteins were down-regulated with the m/z value of 3 708 and 11 856 in recurrence group. Conclusions These differential proteins may play key roles in predicting the prognosis of the patients undergoing liver transplantation for HCC,which may also correspond with the existence of microscopic vascular invasion.
