【摘要】 目的探索建立三甲综合医院与社区卫生服务中心的双向转诊新路径,提高双向转诊的有效性、及时性,提高社区转诊患者的满意度和转诊跟踪知晓度。方法以三甲综合医院四川省人民医院与市区9家设有住院部的社区卫生服务中心为实施对象,以转诊患者作为调查对象,建立在区域医疗卫生服务中心双向转诊创新路径的实施应用,并对社区与综合医院双向转诊应用的效果进行探讨。结果新路径的开发和应用为双向转诊的实施开辟了新的发展方向,为双向转诊的统计及转诊患者数据的采编、上报提供了第一手资料。结论双向转诊服务新路径下社区卫生服务机构与三甲综合医院双向转诊应用研究,可为进一步完善社区与三甲综合医院双向转诊提供规范性、可操作性强的支持依据。更多还原

【Abstract】 Objective Explore the establishment of the top three general hospitals and community health service center two-way referral of new paths.Improve the effectiveness of two-way referral,and enhance community referral patient satisfaction. Methods The top three general hospitals in Sichuan Province People’ s Hospital,with nine inpatient urban community health service centers was investigated.We studied then referral of patients,established the innovative application of the regional health service center two-way referral path and explored the effect of the application of the community and comprehensive two-way referral hospital.Results A new path of development and application implementation for the two-way referral opened a new direction for the two-way referral and referral and statistical summary of the work of editing and reporting patient data to provide first-hand information.Conclusion The applied research of new path two-way referral service agencies and community health services in top three general hospitals provided a normative,and operable to support basis for further improving the community’s top three general hospitals with two-way referral.更多还原
