【摘要】 目的:探讨食管鳞状细胞癌组织中血管、淋巴管生长特点与细胞增殖关系的临床病理意义。方法:应用免疫组化PicTure-Plus法检测62例食管鳞癌石蜡包埋组织Ki-67、CD34和D2-40表达,计数癌巢及其周围功能血管密度(BVD)、微血管密度(MVD)及淋巴管密度(LVD)。结果:62例食管鳞癌组织血管、淋巴管的生长分布存在明显的不均一性,癌周明显多于癌巢组织,且其生长具有明显异质性。MVD在食管下段癌中明显多于中、上段癌,且随组织分化程度的降低而增加、随浸润深度的增加而增加、随S分期的增加而增加,P均<0.05;LVD在淋巴结有转移组明显多于无转移组,SⅢ期明显多于SⅠ期,P均<0.05。Ki-67表达与癌周MVD、BVD和LVD呈正相关,P均<0.05;D2-40在食管鳞癌细胞质中亦有表达,且与癌巢中的MVD、BVD呈正相关,P均<0.05。结论:食管鳞癌组织中癌细胞的增殖与癌周血管、淋巴管的生长呈正相关,血管、淋巴管过度生长是临床易进展的标志之一;Ki-67、D2-40均可作为预后判断的独立检测指标。更多还原

【Abstract】 OBJECTIVE: To investigate the clinicopathological significance of the relation between cell proliferation and the growth characteristics of vessel and lymph-vessel.METODS: To detect the expressions of Ki-67,CD34 and D2-40 and count BVD,MVD and LVD of carcinoma nest and it’s surrounding tissue in 62 cases of paraffin embedding esophageal squamous carcinoma by using PicTure-Plus immunohistochemical method.RESULTS: The growing distribution of vessel and lymph-vessel in 62 cases of esophageal squamous carcinoma was distinct nonhomogeneity,which was higher in surrounding tissue of carcinoma than in carcinoma and was obvious heterogeneity.MVD in inferior segment of esophagus carcinoma was significantly higher than that in middle and superior segment,which was improving with the reducing of tissue differentiation extent and improving with the increasing of infiltrating depth and S stage(all P<0.05).LVD in lymph metastasis group was significantly more than that in non-lymphmetastasis group,which in S Ⅲ was significantly more than in S Ⅰ(both P<0.05).The expression of Ki-67 was positive related with MVD,BVD and LVD(all P<0.05).The expression of D2-40 was positive related with MVD and BVD(both P<0.05),which was detected in cytoplasma of esophageal squamous carcinoma.CONCLUSIONS: The proliferation of carcinoma cell was positive related with the growth of vessel and lymph-vessel in surrounding tissue of esophageal squamous carcinoma,and the overgrowth of vessel and lymph-vessel in carcinoma tissue is a mark of clinical progress.Ki-67 and D2-40 can be both independent detecting index to judge prognosis.更多还原
