【摘要】 目的:探讨术前机械通气对重症法洛四联症(TOF)患儿手术效果的影响。方法:将同期收治的60例TOF患儿随机分为观察组和对照组各30例,对照组患儿术前未进行机械通气而直接手术治疗,观察组患儿术前行气管内插管呼吸机辅助通气。监测机械通气治疗前,治疗后1 h、10 h的血流动力学、动脉血气指标,并且纠正血气和电解质紊乱。结果:手术前观察组机械通气支持治疗后PaO2、SaO2明显升高,PaCO2明显下降,pH值、BE恢复至正常范围,与机械通气前比较差异有统计学意义(P<0.05);与对照组比较,观察组术后机械通气时间明显缩短、手术并发症发生率及病死率明显下降(P<0.05)。结论:术前机械通气支持治疗可以有效纠正缺氧和酸中毒,进而减少婴幼儿TOF术后并发症,提高手术成功率。更多还原

【Abstract】 Objective:To discuss the effect of preoperative mechanical ventilation on the operation effect in the treatment of severe tetralogy of Fallot(TOF) in children patients.Methods:60 children patients with severe TOF were randomly divided into the observation group and the control group(30 cases in each group).The children patients in the control group received operation without preoperative mechanical ventilation and the children patients in the observation group underwent mechanical ventilation via endotracheal intubation.Hemodynamics and arterial blood gas index were monitored one hour and 10 hours before and after treatment,the blood gas and electrolyte disorders were corrected.Results:PaO2,SaO2 significantly increased and PaCO2 decreased after mechanical ventilation support therapy in the observation group before operation,pH and BE returned to the normal range,compared with those before mechanical ventilation(P<0.05);the time of postoperative mechanical ventilation was significantly shorter,the incidence of complications and fatality rate decreased significantly in the observation group,compared with those in the control group(P<0.05).Conclusion:The preoperative mechanical ventilation support therapy can effectively correct hypoxia and acidosis,reduce postoperative complications and increase the successful rate of operation.更多还原
