【摘要】 目的:探讨新辅助化疗联合同期放疗对直肠癌CD44v6表达的影响和意义。方法:采用免疫组化SP法测定36例研究组(术前接受化放疗)及36例对照组(术前未行放化疗)直肠癌患者癌组织、正常组织中CD44v6的表达。结果:两组72例患者正常直肠黏膜上皮细胞中CD44v6表达均阴性。研究组中CD44v6表达显著低于对照组(P<0.05)。两组72例患者中侵润至浆膜层和有淋巴结转移的直肠癌组织CD44v6阳性表达率明显高于侵润至肌层和无淋巴结转移者(P<0.05)。手术并发症两组比较差异无显著性(P>0.05)。研究组保肛率80.6%(29/36)明显高于对照组55.6%(20/36)(P<0.05)。结论:CD44v6在直肠癌的侵袭和转移中起重要作用,检测CD44v6的表达情况是判断直肠癌转移能力的重要指标。新辅助化疗联合同期放疗能有效降低直肠癌组织CD44v6表达水平,从而有效降低了直肠癌细胞的侵袭与转移;同时能使肿瘤降期,提高保肛率,手术并发症无增加。更多还原

【Abstract】 Objective: To investigate effects of neoadjuvant chemotherapy combined with radiotherapy over the same period on expression of CD44v6 in rectal cancer and its clinical significance.Methods: Immunohisto-chemical SP detection was used to examine the expression of CD44V6 on 36 cases of study group(preoperative chemoradiotherapy) and 36 cases of control group(no-line chemotherapy before surgery) in patients with rectal cancer tissue and normal tissue.Results: Two groups of patients with normal rectal mucosa in 72 cases of CD44v6 expression in epithelial cells were negative.CD44v6 study group was significantly lower than the control group(P<0.05).72 cases of patients in both groups to serosal invasion and lymph node positive rectal cancer tissues was significantly higher than CD44v6 invasion to the muscular layer and no lymph node metastasis(P<0.05).Surgical complications between the two groups no significant difference(P>0.05).Sphincter preservation rate of study group 80.6%(29/36)was significantly higher than 55.56%(20/36)(P <0.05).Conclusion: CD44v6 invasion and metastasis in rectal carcinoma play an important role,and detection of the expression of CD44v6 is important to determine indicators of rectal carcinoma metastasis.Neoadjuvant chemotherapy and radiotherapy of the same period can reduce CD44v6 expression in rectal cancer tissues,thus effectively reducing the rectal carcinoma cell invasion and metastasis.Reduction of the tumor can also improve sphincter preservation rates,toxicity can be tolerated,no increase in complications.更多还原
