小剂量右美托咪定对全麻手术患者镇痛镇静效果的影响

【摘要】 目的:探讨小剂量右美托咪定对全麻手术患者镇静、镇痛效果的影响。方法:择期全麻手术患者60例随机分成右美托咪定组(D)和对照组(C),每组30例。D组静脉泵注右美托咪定0.1μg/kg.h,对照组泵注等量的生理盐水。观察术中瑞芬太尼和丙泊酚的用量、血流动力学的变化及其并发症的发生。结果:与组C比较,组D瑞芬太尼和丙泊酚用量明显减少(P<0.05),气管插管和拔管过程中MAP、HR较稳定(P<0.05),高血压、心动过速、呛咳和躁动发生率降低(P<0.05)。结论:小剂量右美托咪定可降低瑞芬太尼和丙泊酚的用量,维持全麻术中血流动力学的稳定,减少并发症的发生。

【Abstract】 Objective:To evaluated the effects of small dose of dexmedetomidine on sedation and analgesia of patients undergoing general anesthesia induction.Methods: A total of 60 patients underwent general anesthesia were randomly divided into the control group(C) and the dexmedetomidine group(D)(n=30).Dexmedetomidine was administered at 0.1 μg/kg.h with a micro infusion pump in group D,and equivalent volume of saline in group C.Observe the dosage of remifentanil and propofol,hemodynamic changes and its complications.Results:The group D required less remifentanyl(P<0.05) and less propofol(P<0.05) compared with the group C.MAP and HR were more stable during endotracheal intubation and extubation process in group D than in group C(P<0.05).The incidence of hypertension,tachycardia,cough and restlessness was lower in group D than group C(P<0.05).Conclusion: Small dose dexmedetomidine is effective in reducing the amount of remifentanil and propofol,maintenancing of hemodynamic stability,and decreasing the incidence of complications.
