【摘要】 目的:探讨3.0T MRI在直肠癌T分期及术前评估中的应用价值。方法:术前经结肠镜活检证实的直肠癌病人32例,行盆腔磁共振扫描,对直肠癌进行影像T分期及术前评估,结果与术后病理对照。结果:32例直肠癌T分期总的准确性单纯平扫为:81.25%(26/32),平扫结合增强为:93.75%(30/32)。32例中28例影像判断筋膜受累阴性者均与术后病理相符,4例影像判断受累阳性,其中2例MR高估。MR矢状位能清晰显示所有病例的肛缘距肿瘤下缘的曲线距离。结论:3.0T MR能较准确地对直肠癌进行术前T分期及评估。更多还原

【Abstract】 Objective:To study the value of3.0T MRI for the Preoperative T Staging and Evaluation of Rectum Cancer.Methods:Thirty-two cases biopsy-proven rectal cancer underwent MR examination before surgery.Preoperative MRI T staging of rectal cancer and postoperative histopathologic T staging were compared.The accuracy of the MRI T staging were caculated.The involvement of mesorectal fascia and the curve distance between the lesion and the analmargin were observed and compared with that of operation and pathology.Results:(1)Accuracy of rectal cancer T staging of non-enhanced and enhanced MR imaging in all 32 cases reached81.25% and93.75%.(2)In evaluating the involvement of mesorectal fascia,two patients were overstepped by MRI.The curve distance between lesions and analmargins could be measured accurately on sagittal section of MR images in all cases.Conclusion:Combination of non-enhanced and enhanced MR imaging can provide accurate rectal cancer T staging.3.0T MR has significant values in evaluating the feasibility of TME(total mesorectal excision)in the patients with rectal cancer.更多还原
