冠心病患者外周血中性粒细胞活化与冠状动脉狭窄程度及心肌损伤标志物的相关性研究

【摘要】 目的:探讨冠心病患者外周血中性粒细胞(PMN)活化与冠状动脉狭窄程度及肌钙蛋白T(CTnT)的关系;方法:82例冠心病患者,包括38例急性心肌梗死(AMI)、23例不稳定型心绞痛(UA)、21例稳定型心绞痛(SA)患者、30例正常对照,采用流氏细胞技术检测中性粒细胞粘附分子CD11b/CD18和CD62L,同时作冠状动脉造影定量分析冠状动脉病变积分(CAS),以评价冠状动脉狭窄程度,并测量心肌肌钙蛋白T。结果:冠心病患者存在CD62L表达的下调和CD11b/CD18表达的上调,AMI患者比UA患者明显,UA患者比SA患者明显,而SA患者仅在CD11b/CD18表达上高于正常组;CTnT阳性与阴性患者相比,其CD62L表达下降而CD11b/CD18表达增高;CAS在SA、UA与AMI患者间,CTnT阳性组与阴性组间均无明显差异;相关性分析显示:CTnT与CD62L成负相关、与CD11b/CD18成正相关,且相关性较好,CD62L与CD11b/CD18成负相关,而CAS与CD62L及CD11b/CD18的相关性无统计学意义。结论:在冠心病患者尤其是UA和AMI患者中,PMN是处于活化状态的,尤其是PMN表面细胞粘附分子CD62L和CD11b/CD18,在冠心病患者尤其是急性冠脉综合征的发生发展中起到了重要作用。

【Abstract】 Objective:To investigate the relation between polymorphonuclear neutrophil(PMN) activation and coronary artery score and troponin T(cTnT) of coronary artery disease(CAD).Methods:82 CAD patients including 23 unstable angina pectoris(UA),38 acute myocardial infarction(AMI) and 21 stable angia pectoris(SA).CD62L and CD11b/CD18 on polymorphonuclear neutrophils were analyzed by flow cytometry.The severity of coronary artery lesions was quantitative analysed by coronary arteriongraphy,coronary artery score(CAS) was calculated, and CTnT were detected.Results:Expression of CD62L was significantly lower and CD11b/CD18 higher in CAD patients,and they were more significant in AMI than in UA,and similar results were more often found in UA than in SA;and there was similar change between CTnT positive and CTnT negative;and no difference in CAS between UA and AMI,CTnT positive and cTnT negative.There was notably negative correlation between CTnT and CD62L,CTnT and CD62L,positive correlation between cTnT and CDllb/CD18;and no correlation between CAS and CD62L,CAS and CD11b/CD18.Conclusion:There exists PMN activation among CAD patients,and the detection of CD62L and CD11b/CD18 may play an important role in the development of CAD.
