上转发光免疫层析法定量测定HBsAg试剂条在血液筛检中的应用
Evaluation of Quantitative Immuno inflorescent Chromatography Assay Used in Blood Screen for HBsAg
【摘要】 目的评价上转发光免疫层析法HBsAg定量测定试纸条在无偿献血者献血前血液筛检中的应用效果。方法应用卫生部临床检验中心的HBsAg血清盘对ELISA、上转发光法和胶体金法检测HBsAg的灵敏度、敏感性、特异性和符合性等进行测试;用本站ELISA-HBsAg检测为阳性或可疑的无偿献血者标本27份、医院乙型肝炎患者35份和本市固定无偿献血者的29份,同时进行ELISA、胶体金法和上转发光法检测HBsAg。结果 ELISA检测血液中HBsAg灵敏度为0.2IU/ml,敏感性为88.46%,特异性为100.00%,符合性92.50%;上转发光法检测血液中HBsAg灵敏度为0.5IU/ml,敏感性为76.92%,特异性为100.00%,符合性85.00%;胶体金法检测血液中HBsAg灵敏度为3IU/ml,敏感性为42.31%,特异性为100%,符合性72.50%。结论上转发光法定量测定试纸条检测血液HBsAg的灵敏度是胶体金法检测的6倍,敏感性和符合性均远高于胶体金法,且灵敏度接近ELISA法,所以上转发光法HBsAg定量测定试纸条适合无偿献血者献血前的血液初筛。
【Abstract】 Objective To evaluate a new method of immuno inflorescent chromatography assay used for blood donor screen for HBsAg.Methods Three reagents including ELISA HBsAg,gold immunochromatography assay and immuno inflorescent chromatography assay were used to test national HBsAg panel to determine the specificity and sensitivity of these methods.A clinical study was also performed to compare the sensitivity and specificity of these methods by using 27 clinical samples from blood bank,35 clinical samples from hospital and 29 clinical samples from blood donors.Results The minimum detectable concentration was 0.2 IU/ml for ELISA HBsAg,3 IU/ml for gold immunochromatography assay,0.5 IU/ml for immuno inflorescent assay.The sensitivity was 88.46% for ELISA HBsAg,76.92% for gold immunochromatography assay,42.31% for immuno inflorescent assay.The specificity was 100.00% for all the methods which were compared;The accuracy is 92.50% for ELISA HBsAg,85.00% for Gold immunochromatography assay,72.52% for immuno inflorescent assay.Conclusion The sensitivity of immuno inflorescent assay is much higher than that of gold immunochromatography assay,and is similar than that of ELISA,so the new method of immuno inflorescent assay is suitable to be used in blood donor screen.
