帕金森病的黑质超声表现

【摘要】 目的研究帕金森病(PD)患者的黑质超声表现及其影响因素。方法采用连续入组的方法 ,选择PD患者53例为PD组,另选同期年龄、性别相匹配的53例为对照组,行经颅实时彩色多普勒超声(TCS)检测,描记黑质回声面积,根据回声面积的大小对黑质回声分级。比较2组黑质高回声的比例,并且分析其影响因素。结果 PD组黑质高回声所占的比例较对照组明显升高(P<0.01);黑质高回声与患者的年龄、发病年龄、病程及疾病的严重程度无相关性(P>0.05);黑质回声面积诊断PD价值的ROC曲线下面积为0.910±0.029,取黑质回声面积为0.20,黑质高回声诊断PD的敏感性85%,特异性78%,准确性81%。结论 TCS检测到的黑质高回声可能是PD患者特征性的表现;用ROC曲线评价黑质回声面积对PD的诊断价值较大;TCS检测的黑质高回声在健康人群中的意义有待进一步研究。

【Abstract】 Objective To study the substantia nigra(SN) ultrasonography findings in patients with Parkinson’s disease(PD) and its influencing factors.Methods Fift-three PD patients served as a PD group and 53 individuals with their age and sex matched served as a control group.Their SN hyperechogenicity was measured by cranial real time color Doppler ultrasonography and graded. SN hyperechogenicity ratio was compared between the two groups and its influencing factors were analyzed.Results The SN hyperechogenicity ratio was significantly higher in PD group than in control group(P<0.01).The area under ROC curve was 0.910±0.029 when the the SN hypere-chogenicity was used to diagnose PD.The severity,specificity and accuracy of SN hyperechogenicity were 85%,78%and 81%respectively when the SN hyperechogenicity was 0.20.Conclusion The SN hyperechogenicity detected by cranial real time color Doppler ultrasonography is the characteristic finding in PD patients.The ROC curve is of a rather good value for the diagnosis of PD with SN hyperechogenicity.Further study is needed on the SN hyperechogenicity detected by cranial real time color Doppler ultrasonography.
