【摘要】 目的观察小梁切除术联合羊膜移植术治疗青光眼的临床效果。方法选择需行滤过手术的58例(58只眼)青光眼患者,随机分为小梁切除术组(对照组)29例(29只眼)和小粱切除联合羊膜移植术组(实验组)29例(29只眼)。所有患者随访12个月,分析比较两组术后的降眼压效果、滤过泡形成情况及并发症。结果术后12个月时实验组平均眼压(14.7±3.6)mm Hg,对照组平均眼压(16.4±5.0)mm Hg,两组之间的平均眼压差异无统计学意义(P=0.143);实验组手术成功率(93.1%),对照组手术成功率(82.8%),差异无统计学意义(校正检验P=0.420);功能性滤过泡实验组(89.7%)较对照组(65.5%)多,差异有统计学意义(P=0.028);术后浅前房发生率实验组(13.8%)明显低于对照组(37.9%),差异有统计学意义(P=0.036)。结论小梁切除术联合羊膜移植可以有效的治疗青光眼,成功率高。更多还原

【Abstract】 Objective To compare outcomes of trabeculectomy combined with mitomycin C(MMC) and amniotic membrane transplantation(AMT) with those of trabeculectomy with MMC alone in glaucoma.Methods This prospective randomized clinical trial included 58 eyes with glaucoma.Trabeculectomy with MMC and single-layer AMT under the scleral flap was performed in 29 eyes(study group) and trabeculectomy with MMC alone in 29 eyes(control group).The outcome measures included intraocular pressure(IOP) and appearance of the bleb were compared and complication between the two groups.All patients were followed for 12 months.Results The difference of the mean postoperative intraocular pressure was statistically significant in the control group and in the study group at 12 months follow-up(P<0.05).IOP decreased from 37.4±6.3 mm Hg and 35.6±8.4 mm Hg preoperatively to 16.4±5.0 mm Hg and 14.7±3.6 mm Hg postoperative respectively.However,there was no IOP difference between the two groups postoperatively at 12 months(P=0.143).Success rates of the study group(IOP≤21 mm Hg or with glaucoma medications) is 93.1%,And the control group is 82.8%,There were not significant difference among the two groups(P=0.420).Promote forming functiona filtering bleb in the study group(89.7%) is more than in the control group(65.5%),the difference was statistically significant(P=0.028).Postoperative shallow anterior chamber e study group incidence(13.8%) was significantly lower than the control group(37.9%),the difference was statistically significant(P=0.036).Conclusion Trabeculectomy combined with MMC and AMT has higher success rates is worth further clinical popularization and application.更多还原
