【摘要】 目的探讨羊膜移植联合曲安奈德治疗复发性翼状胬肉的临床疗效及其术后复发情况及泪膜、视力变化。方法选择超过瞳孔缘32例(32只眼)复发性翼状胬肉患者,在手术显微镜下切除翼状胬肉,做广泛的球筋膜切除,脱水羊膜复水,浸润于40 mg/1ml曲安奈德混悬液中5 min后移植。术后随访12～36.2个月,中位数15.6个月,观察患者术前、后视力变化和术后复发情况。术前、后行泪液分泌试验(SIt)、泪膜破裂时间(BUT)检查。结果患者术前裸眼视力指数/眼前至0.8,中位数为0.3。术后3个月裸眼视力指数/眼前至1.2,中位数为0.6(秩和检验U=5.231,P<0.01),1例患者术后复发,复发率9.4%。术后3个月时SIt较术前无明显变化(P>0.05),BUT较术前明显增加(P<0.05)。结论羊膜移植联合曲安奈德治疗复发性翼状胬肉安全、有效,术后复发率低,视力提高显著,患者眼表舒适度增加。更多还原

【Abstract】 Objective To evaluate the efficiency and safty of amniotic membrane transplantation and triamcinolone acetonide for treatment of recurrent ptrygium.Methods Thirty-two eyes with recurrent pterygium which invading the cornea over the pupil border were included.recurrent pterygium was dissected under surgical microscope with tansplanting amniotic membrane and triamcinolone acetonide.degenerative Tenon’s capsule was removed totally and the wound was covered by amniotic membrane.these patients were followed-up for 12.0~36.2 months(median:15.6 months).changes in visual acuity and recurrence rate after operation were observed.SchirmerⅠtest(SIt),Break up time of the tear film(BUT) were performed at pre-operation and post-operation.Results Non-corrected visual acuity averaged 0.3(ranged from finger count to 0.8)before the operation and averaged 0.6(ranged finger count to 1.2)3 months postoperatively(Wilcoxon signed rank test μ=5.231,P<0.01).Ptrygium relapsed in 1 eye with a recurrence rate of 9.4%(1/32).there was a no significant difference of SIt(P>0.05) but significant increase of BUT(P<0.05).Conclusion The amniotic membrane transplantation and triamcinolone acetonide was an effetive and safe method to treat recurrent ptrygium更多还原
