氨基末端脑钠肽前体对急性呼吸困难的鉴别诊断意义

【摘要】 目的:探讨氨基末端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)对急性呼吸困难的鉴别诊断意义。方法:将426例急性呼吸困难患者分为心源性呼吸困难组(n=206)和肺源性呼吸困难组(n=220),分别测定患者血浆NT-proBNP浓度。结果:心源性呼吸困难组血浆NT-proBNP(2351.67±368.61 pg/m L)浓度高于肺源性呼吸困难组(243.54±57.18 pg/m L),差异有统计学意义(P<0.01);心源性呼吸困难组不同年龄亚组血浆NT-proBNP浓度的差异有统计学意义(P<0.05),心源性呼吸困难血浆NT-proBNP浓度在心功能不同级别间差异有统计学意义(P<0.01)。结论:测定血浆NT-proBNP浓度对于鉴别心源性呼吸困难和肺源性呼吸困难具有较高的应用价值。

【Abstract】 Objective:To investigate the value of N-terminal pro-brain natriuretic peptide(NT-proBNP) in the differential diagnosis of actue dyspnea.Methods:A total of 426 patients with actue dyspnea were divided into two groups: cardiac dyspnea group(206 cases)and pulmonary disease group(220 cases).Plasma NT-proBNP levels of the patients were detected.Results: The NT-proBNP levels of the patients with dyspnea caused by heart failure was obviously higher than that of the patients with dyspnea caused by pulmonary disease(P<0.01).The levels of NT-proBNP were significantly different among patients of different ages(P<0.05),as well as patients of different cardiac function grades(P<0.01),in cardiac dyspnea group.Conclusion: Plasma NT-proBNP level is of great value in diffential diagnosis of acute dyspnea.
