微种植支抗移位的影响因素探讨
Assessment of factors resulting mini-implant displacement
【摘要】 目的:探讨正畸治疗中影响微种植支抗位移的因素。方法:选择26例拔除上颌第一前磨牙的安氏Ⅱ1类正畸病例共52颗微种植钉作为研究对象,男5例,女21例,年龄14~25岁。在上颌第一磨牙和第二前磨牙牙根之间植入微种植钉作支抗,随机选择加载时机、施力方式和施力的初始力值,根据临床需要选择力作用线与牙合平面夹角和施力持续时间。分别在施力前后拍摄头颅侧位定位片,测量种植钉头部在施力前后的位置变化。通过单个样本的t检验比较施力前后种植钉的位移量变化;用独立样本t检验比较不同性别及不同施力方式对种植钉的位移量的影响;以种植钉的位移量(D)为应变量,以加载开始时间(W)、施力持续时间(T)、施力的初始力值(F)、力作用线与牙合平面夹角(A)、年龄(Y)为自变量,通过多元逐步回归法建立多元回归方程,用以分析各自变量对应变量的影响作用。结果:施力后种植钉的位移量为0.42±0.28 mm(P<0.05),性别和施力方式对种植钉位移量的影响无统计学意义(P>0.05)。自变量与应变量依存关系的多元回归方程为:D=0.5467-0.0751W+0.0168T-0.0219Y。结论:施力后微种植支抗可产生位移,位移量的大小与加载开始时间、施力持续时间和年龄有关。
【Abstract】 Objective:The purpose of this study was to assess factors led to displacement and quantitative evaluation of orthodontic mini-implant displacement.Method:52 orthodontic mini-implants was placed in 26 patients of class Ⅱdivision 1 of maxillary first premolar extraction for orthodontic reasons.They(5 males and 21 females) aged from 14 to 25.Orthodontic mini-implants(n=52) were placed between the upper second premolars and first molars.Randomly choose the loading method,power of the stress,the lasting time,the loading time and angle between occlusal plane and force active line.The preretraction and postretraction cephalometric films were measured to evaluate the positional difference of implants of the sides.The results were analyzed with a t-test for the study of the effect on data between preretraction and postretraction.From the two independent sample t-test we could observe the effect of different gender and different loading method on mini-implant displacement.The results were analyzed with independent variable of variance for the quantity of the starting time(W),initial load application(F),lasting time(T),age(A) and the angle between occlusal plane and force active line(A),and with dependent variable of mini-implant displacement data(D),for the study of these factors influencing mini-implant displacement.Result:Displacement of mini-implants was 0.42±0.28 mm and was significantly affected(P < 0.05).While there were no significant(P > 0.05) differences in displacement of mini-implants associated with gender and loading method.The multiple linear regression equation was D=0.5467-0.0751W +0.0168T-0.0219Y.Conclusion:Load application led to displacements that were associated with age,the time of initial load application,and the lasting time.
