颅内神经鞘瘤与脑膜瘤的磁敏感加权成像表现及比较
Susceptibility Weighted Imaging in Differential Diagnosis Between Schwannoma and Meningioma
【摘要】 目的探讨颅内神经鞘瘤与脑膜瘤的磁敏感加权成像(SWI)表现,从肿瘤内微出血的角度评估SWI区分神经鞘瘤与脑膜瘤的价值。方法 2008年至2011年于本院手术,经病理证实的42例神经鞘瘤与31例脑膜瘤患者入组,患者术前均行常规磁共振扫描及SWI。评估并比较二者肿瘤内磁敏感性低信号(ITSHIA)发生率及IT-SHIA频数评分的差异。结果神经鞘瘤瘤体内ITSHIA的发生率明显高于脑膜瘤(χ2=40.958,P=0.000)。神经鞘瘤瘤内ITSHIA的半定量评分明显高于脑膜瘤(Zc=6.088,P=0.000)。结论 SWI能够评估肿瘤内部结构和病理特征。由于微出血及其代谢产物的敏感性使SWI成为鉴别诊断神经鞘瘤与脑膜瘤的有效序列。在除外钙化的情况下,该序列可作为常规序列的补充。
【Abstract】 Objective To explore the imaging characteristics of the schwannomas and meningoma on SWI.Methods 42 patients with schwannoma and 31 patients with meningioma confirmed by surgery and pathology were analyzed retrospectively.All cases received conventional MRI and SWI sequence preoperatively.Intratumor susceptibility hypointensity(ITSHIA) data of different tumors were compared and analyzed.Results Obvious difference existed in the semiquantitative data of ITSHIA among two different tumors.The appearance frequency of ITSHIA within schwannomas were significantly higher than that in meningioma(χ2=40.958,P=0.000).The values of ITSHIA frequency score of schwannoma were higher than that of meningioma(Zc=6.088,P=0.000).Conclusion SWI is an effective sequence in differential diagnosis between schwannoma and meningioma owing to its sensitivity in microbleeding.SWI can be used as a supplementary of conventional sequence in the situation without calcification by CT.
