重症监护室呼吸机相关性肺炎病原菌分布及耐药性监测
Monitoring on the distribution and drug resistance of ventilator-associated pneumonia pathogens in ICU
【摘要】 目的分析呼吸机相关性肺炎的常见病原菌及耐药性监测。方法调查本院重症监护室(ICU)106例呼吸机相关性肺炎患者的下呼吸道标本,分离培养鉴定菌株并行药敏试验。结果在分离出的168株菌株中,革兰氏阴性杆菌(Gramnegative bacilli,GNB)123株(73.21%),革兰氏阳性球菌(Gram-positive bacilli,GPB)36株(21.42%);真菌9株(5.36%)。铜绿假单胞菌和金黄色葡萄球菌占比最大,分别为21.30%和19.64%。绝大部分GNB呈现较强的耐药性。结论 ICU患者感染及多药耐药严重,加强耐药性监测,合理使用抗菌药物十分重要。
【Abstract】 Objective To explore the pathogens causing ventilator-associated pneumonia(VAP) in an intensive care unit(ICU) and their drug resistance so that it could provide theoretical foundation for antibiotics use.Methods 106 patients with VAP in ICU were investigated and the sputa and secretions of lower respiratory tract were cultured separately for drug sensitive test.Results The majority of 168 isolated pathogen strains was gram-negative bacilli(123 strains),and the rest were gram-positive bacilli(36 strains) and fungus(9 strains),in which pseudomonas aeruginosa took 21.30% and staphylococcus aureus 19.64%.Most pathogenic bacteria showed strong drug resistance.Conclusion The infection and multi-drug resistance of pathogens are serious in ICU,therefore it′s important and urgent to carry out surveillance of bacterial resistance for antibiotics use.
