非小细胞肺癌患者Th1/Th2型细胞因子的表达及其临床意义
Expressions of Th1/Th2 Cytokines in the Peripheral Blood of Patients with Non-Small Cell Lung Cancer and Its Clinical Significance
【摘要】 目的研究辅助性T淋巴细胞(helper T lymphocyte,Th)1、2型细胞因子在非小细胞肺癌(non-small cell lung canc-er,NSCLC)患者外周血中的表达。方法采用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测40例NSCLC患者手术前后和20例正常对照外周血中Th1型细胞因子白细胞介素-2(interleukin-2,IL-2)、肿瘤坏死因子-α(tumor necrosis fac-tor-α,TNF-α)和Th2型细胞因子IL-10、转化生长因子-β1(transforming growth factor-β1,TGF-β1)的水平。结果 NSCLC组术前IL-2、TNF-α水平较正常对照组明显降低(P<0.05);IL-10、TGF-β1水平则明显升高(P<0.05)。NSCLC患者手术后IL-2、TNF-α水平较手术前明显升高(P<0.01);IL-10、TGF-β1水平则明显降低(P<0.01)。外周血Th1/Th2型细胞因子水平与NSCLC临床病理无关。结论①NSCLC患者呈现Th1抑制和Th2极化状态,是肺癌发生免疫耐受的可能机制;②手术切除肿瘤可在一定程度上逆转Th1/Th2的失衡状态,改善机体免疫状态,对预后产生积极作用。
【Abstract】 Objective To investigate the changes of helper T lymphocyte(Th) cytokines in the peripheral blood of patients with non-small cell lung cancer(NSCLC),and their relationships with clinical pathology and operation therapy of NSCLC.Methods The expressions of the Th1/Th2 cytokines including Th1: interleukin-2(IL-2),tumor necrosis factor-α(TNF-α),Th2: interleukin-10(IL-10),transforming growth factor-β1(TGF-β1) in the serum samples from 40 patients with NSCLC before and after operation and 20 healthy controls were detected by enzyme-linked immunosorbent assay(ELISA).Results The levels of IL-2,TNF-α in NSCLC group were significantly lower than those in healthy control group(P<0.05),while the levels of IL-10,TGF-β1 were significantly higher(P<0.05).The levels of IL-2,TNF-α in NSCLC patients after operation were significantly higher than that in NSCLC patients before operation(P<0.01),while the levels of IL-10,TGF-β1 were significantly lower(P<0.01).The expressions of the Th1/Th2 cytokines had no correlations with clinicopathologic characteristics of NSCLC.Conclusion ① There is a tendency of Th1 suppression and Th2 polarization in NSCLC patients,which may be a mechanism of immunotolerance with NSCLC;② To some extent,surgical removal of tumors can reverse the Th1/Th2 imbalance,improve the immune status,and has a positive effect on the prognosis.
