心肌超声造影评价冠心病早期微循环损伤的应用研究
Evaluation of myocardial contrast echocardiography on the early injury of myocardial microcirculation in coronary heart disease
【摘要】 目的探讨心肌超声造影(MCE)在冠心病早期冠状动脉微循环灌注异常中的应用价值。方法选择冠状动脉造影检查结果正常的冠心病高危人群20例(A组)和年龄匹配的健康志愿者20例(B组),分别行MCE并4次闪烁成像,比较两组受检者闪烁成像后心肌微循环再充盈峰值强度(PI)、达峰值强度时间(TP)及微泡持续显影时间(T)的差异并作定量分析。结果 A组闪烁成像后心肌微循环再灌注PI为(-30.4±7.3)dB,TP为(12.6±5.2)s,T为(7.6±2.2)min,B组PI为(-21.5±8.6)dB,TP为(8.2±4.8)s,T为(4.1±1.9)min,两组比较差异有统计学意义(均P﹤0.05)。结论应用MCE多点闪烁显像,通过监测心肌微循环再充盈PI、TP及T可达到定量评价冠心病早期心肌微循环灌注异常的目的,有重要临床应用价值。
【Abstract】 Objective To evaluate the value of myocardial contrast echocardiography (MCE) on myocardial microcirculation perfusion of coronary heart disease at early stage. Methods Twenty subjects with angiography normal coronary arteries were involved in group A and twenty healthy volunteers were taken as the control (group B). Realtime myocardial contrast echocardiography was performed and flash imaging was applied to evaluate myocardial microcirculation perfusion. The peak video intensity(PI),the time to peak concentration(TP) and the microbubble duration time(T) of two groups were recorded for analysis. Results The PI in group A was lower than that in group B [(-30.4±7.3) dB vs. (-21.5±8.6) dB,P﹤0.05]. The TP and T in group A were longer than those in group B [(12.6±5.2) s vs. (8.2±4.8) s;(7.6±2.2) min vs. (4.1±1.9) s,P﹤0.05].Conclusion With the application of MCE technology,coronary microcirculation perfusion abnormalities in patients with early coronary heart disease can be quantitatively evaluated by monitoring the PI,TP,T of microcirculation.
