观察超声弹性成像定量分析技术在睾丸微石症诊断中的应用价值

【摘要】 目的评价超声弹性成像(UE)定量分析技术在睾丸微石症(TM)诊断中的应用价值。方法对10例TM患者(TM组)及50名健康男性志愿者(正常对照组)行UE检查,对比分析TM组及正常对照组睾丸组织的硬度、均匀程度及UE声像图表现的差异。观察反映睾丸组织硬度及均匀程度的UE定量分析指标,包括双侧睾丸应变率比值、同侧睾丸实质与附睾头应变率比值及睾丸与附睾头应变均值的差值。结果正常睾丸UE声像图表现为"红色周边带-蓝色边缘区域-绿色中心区域"。TM患者睾丸UE声像图表现与正常睾丸相似,睾丸微小钙化表现为点状强回声。UE定量分析结果显示两组间UE定量分析指标的差异均无统计学意义(P均>0.05)。结论 UE检查可较为准确地反映正常睾丸组织的硬度及均匀程度。但目前UE技术受信号获取精度的限制,其对TM的诊断价值有待商榷。

【Abstract】 Objective To observe the application value of ultrasound elastography(UE) with quantitative analysis technique in diagnosing testicular microlithiasis(TM).Methods Totally 10TM patients(TM group) and 50health male adults(normal control group) were enrolled and examined with UE.A contrastive analysis was performed in order to study the difference in UE features,hardness and evenness degree of the testes between the two groups.The quantitative analysis indexes included the strain rate between the left and right sides of testicles,the strain rate and the difference value of the mean strain between homolateral parenchyma of testis and caput epididymidis.Results Testicles on UE sonogram manifested as an area with green in the central region,blue in the lateral side and red around the borderline in normal control group.The three-layer structure of the testicles similar to normal control group was also observed in TM group.The minor calcifications in testicles were shown as punctate hyperechoic focus.There was no significant difference of the above 3quantify analysis indexes between the two groups(all P>0.05).Conclusion UE can be used to describe the hardness and evenness degree of the testes.But being limited by the accuracy of current data acquisition,the diagnostic value of UE in TM should be further discussed.
