【摘要】 目的:探讨几丁糖联合中药保留灌肠防治介入治疗输卵管复通后再粘连的临床疗效。方法:选择98例输卵管近端梗阻患者,随机分为对照组和治疗组,两组患者月经干净后3～5 d介入治疗实施复通术,复通成功者可经导管向输卵管腔内缓慢注药(庆大霉素8万U2、%利多卡因2 mL、地塞米松5 mg、α-糜蛋白酶4 000 U、生理盐水5 mL后再注入几丁糖15 mL)防治粘连。治疗组患者术后除经期外每日行中药保留灌肠(方药组成:紫花地丁、红藤、蒲公英、败酱草各30 g,路路通、王不留行子、皂角刺、丝瓜络各20 g,三棱、莪术各15 g,黄柏15 g,昆布、海藻、浙贝母各15 g)。结果:对照组再粘连率为30.0%,宫内妊娠率为8.33%;治疗组再粘连率为11.11%,妊娠率为18.00%,两组比较差异有统计学意义。结论:几丁糖联合中药保留灌肠可有效减少介入治疗输卵管复通术后再粘连,提高复通术后的妊娠率。更多还原

【Abstract】 Objective:To explore the clinical value of the combination of tetrose and Chinese medicine retention enema in the prevention and interventional treatment of repeated adhesion after fallopian tube recanalization.Methods:98 cases of proximal fallopian tube obstruction were randomly divided into the control group and the treatment group.Patients of two groups were treated by recanalization operation through interventional treatment after 3 to 5 days of the end of menstruation,and then patients of successful recanalization were slowly injected medicine(tetrose 15 mL and the mixture of gentamicin eighty thousands U,2% lidocainum 2 mL,dexamethasone 5 mg,α-chymotrypsin 4 000 U,saline solution 5 mL and tetrose 15 mL) in fallopian tube cavity through catheter to prevent adhesion.Patients of the treatment group were given Chinese medicine retention enema every day except menstruation after operation(composition of enema prescription:Viola ycdoensis 30 g,Caulis Sargentodoxae 30 g,Taraxacum mongolicum 30 g,Herba Thlaspis 30 g,Liquidambar formosana Hance 20 g,Gleditsia sinensis Lam 20 g,Luffa cylindrica 20 g,Sparganium stoloniferum 15 g,Curcuma aeruginosa Roxb 15 g,Phellodendron amurense Rupr 15 g,Laminaria japonica Aiesch 15 g,Algae15 g Fritillaria thunbergii 15 g).Results:Repeated adhesion rate of the control group and the treatment group was 30.00% and 11.11% respectively;uterine pregnancy rate of the control group and the treatment group was 8.33% and 18.00% respectively and the difference had statistical significance.Conclusion:The combination of tetrose and Chinese medicine retention enema can effectively decrease repeated adhesion after fallopian tube recanalization operation,and improve the pregnancy rate.更多还原
