北京市某城区居民缺血性心脏病死亡与大气表观温度之间的相关性研究
Association between Ambient Apparent Temperature and Cause-specific Mortality of Ischemic Heart Diseases in Beijing,China
【摘要】 目的探讨北京市某城区居民缺血性心脏病死亡与表观温度之间的相关性。方法收集2004—2008年北京市某城区气象因素、大气污染物水平及缺血性心脏病死亡数据,以缺血性心脏病每日死亡人数为因变量,通过平滑函数控制死亡的长期趋势和短期波动以及大气污染物水平、气象因素对表观温度的混杂作用,使用时间序列分析的广义相加模型拟合表观温度与缺血性心脏病死亡人数间的暴露-反应关系模型,分析该城区居民缺血性心脏病死亡与表观温度之间的关系。结果该城区2004—2008年缺血性心脏病死亡6 664人,其中男性3 559人(53.41%),女性3 105人(46.59%),75岁以上4043人(60.67%)。日均表观温度与缺血性心脏病死亡的关系基本呈现"U"字型,日均表观温度在高温时的上升及在低温时的下降均会造成缺血性心脏病死亡的增多;日均表观温度≥12℃时,日均表观温度每上升1℃的全人群超额死亡率为1.202%(95%CI:0.123%~2.293%),男性为0.279%(-1.210%~1.790%),女性为1.807%(0.238%~3.401%);日均表观温度<12℃时,日均表观温度每下降1℃的全人群超额死亡率为1.687%(0.623%~2.740%),男性为2.619%(1.197%~4.021%),女性为1.764%(0.189%~3.314%)。结论不同分层的日均表观温度与缺血性心脏病死亡的关系不同,低温比高温的影响大,低温时表观温度的影响存在性别差异。
【Abstract】 Objective To explorer the relationship between ambient apparent temperature and ischemic heart disease mortality in one district of Beijing.Methods The data of meteorological factors,air pollutants(SO2,N02 and PM10)and ischemic heart disease(IHD) mortality in one district of Beijing from 2004 to 2008 were collected and the generalized additive model were fitted with use of natural cubic spline function to adjust trends in time,day of week,air pollutants and other meteorological factors.Results A total of 6 664 IHD death cases were collected,included 3 559(53.41%) male,3 105(46.59%) female and 4 403(60.67%) of older people(≥75 years).The graphical associations between ambient apparent temperature and IHD mortality were observed"U"shape.Both decrease at lower temperature and increase at higher temperature of ambient apparent temperature(AAM) can cause more IHD death.Each 1℃decrease in AAM when AAM≥12 was associated with an increase in IHD mortality of 1.202%(95%CI:0.123%-2.293%) for all,0.279%(95%CI:-1.210%-1.790%) for male,1.807(95%CI:0.238%- 3.401%) for female.Each 1℃decrease in AAM when AAM<12 associated with an increase in IHD mortality of 1.687%(95%CI: 0.623%-2.740%) for all,2.619%(95%CI:1.197%-4.021%) for male,1.764(95%CI:0.189%-3.314%) for female.After 24,48 and 72 h treated,compared with the negative control,the cell survival rates of 64,128μg/ml group decreased significantly(P<0.05),and the DNA Olive tail moment of all groups increased significantly(P<0.05).Compared with the 24 h group,the cell survival rates of all treated groups in 72 h decreased significantly(P<0.05),and the DNA Olive tail moment of all groups in 48 and 72 h increased significantly(P<0.05).Conclusion Stratified analysis shows different associations between ambient apparent temperature and ischemic heart disease mortality.Lower ambient apparent temperature causes stronger adverse effect than higher temperature.Gender causes different effect when ambient apparent temperature is lower.
