【摘要】 目的观察氯诺昔康复合丙泊酚用于合并矽肺病患者无痛胃镜检查时的安全性及可靠性。方法选取249例合并矽肺病患者,随机分为丙泊酚组（A组）、丙泊酚+芬太尼组（B组）及丙泊酚+氯诺昔康组（C组）,每组83例。记录麻醉起效时间、手术持续时间、麻醉清醒时间,注药前、注药2min后SBP、DBP、HR、SpO₂变化,重点观察SpO₂变化。记录丙泊酚用量,术中麻醉镇痛效果,术中知晓情况及术后麻醉满意程度。结果 B组、C组麻醉起效时间、清醒时间、丙泊酚用量与A组相比（P<0.05）,B组与C组相比（P>0.05）。注药2min后A组患者SBP、DBP、HR与用药前相比（P<0.01）,B组与用药前相比（P<0.05）,C组与用药前相比（P>0.05）。注药2min后A组SpO₂变化与注药前相比（P<0.05）,B组与注药前相比（P<0.01）,C组与用药前相比（P>0.05）。术中麻醉镇痛效果达优的例数、术后麻醉满意度达优的例数及术中知晓的例数B组、C组与A组相比（P<0.05）,B组、C组2组相比（P>0.05）。结论氯诺昔康复合丙泊酚用于合并矽肺病患者行无痛胃镜检查是安全可靠的。更多还原

【Abstract】 Objective To observe the safety and reliability of lornoxicam combined with propofol in painless gastroscopy for patients complicafed with silicosis.Methods The 249 patients complicafed with silicosis were randomly divided into propofol group（group A）,propofol +fentanyl group（group B）,propofol + lornoxicam group（group C）,with 83 patients in each group.The onset time of anesthesia,operation duration,anesthesia recovery time were recorded,and SBP,DBP,HR,SpO₂ before injection and 2min after injection were detected and observed,especially for the changes of SpO₂.The usage amount of propofol,the effect of anesthesia and analgesia during operation,intraoperative awareness degree and satisfaction degree for the effect of anesthesia were recorded and analyzed.Results There were significant differences in the onset time of anesthesia,anesthesia recovery time and usage amount of propofol between group B,group C and group A（P<0.05）,so were between group B and group C（P>0.05）.There were significant differences in the changes of SBP,DBP,HR before injection and 2min after injection in group A（P<0.01）,so were in group B（P<0.05） and group C（P<0.05）.There was a significant difference in the change of SpO₂ before injection and 2min after injection in group A（P<0.05）,so were in group B（P<0.01） and group C（P<0.05）.There were significant differences in the cases of excellent effect of anesthesia and analgesia,the cases of well satisfactory degree after operation and cases of intraoperative awareness between group B,group C and group A（P<0.05）,so were between group B and group C（P<0.05）.Conclusion Lornoxicam combined with propofol is safe and reliable in carrying out painless gastroscopy for patients complicafed with silicosis.更多还原
