【摘要】 目的:探讨腰椎管狭窄症围手术期镇痛的方法与帕瑞昔布镇痛效果。方法:收集广州市第一人民医院脊柱外科2011年7-12月的腰椎管狭窄症手术患者共82例。分为空白对照组35例、单次注射帕瑞昔布组27例和两次注射帕瑞昔布组20例。所有患者术前、术后1、3、5 d进行疼痛评分(visual analog scale score,VAS)和生活质量评分(activity of daily living,ADL),并进行对照比较。结果:单次和2次注药组在术后1、3、5 d时的疼痛明显低于空白对照组,生活质量评分高于空白对照组,P<0.05;术后1d两次注药组疼痛程度低于单次注药组(P<0.05),术后3 d、5 d差异无统计学意义(P>0.05)。生活质量评分两次注药组在术后1和3 d时好于单次注药组(P<0.05),术后5 d时,两者差异无统计学意义(P>0.05)。结论:帕瑞昔布对腰椎管狭窄症围手术镇痛有良好的效果;术后1 d注药使用2次,以后2 d各使用1次,可起到良好的镇痛作用。更多还原

【Abstract】 Objective:To investigate the approaches for perioperative analgesia and effects of parecoxib for lumbar spinal stenosis.Methods:Patients underwent surgeries of lumbar spinal stenosis between July 2011 and December 2011 were recruited form the department of spinal surgery,Guangzhou First People’s Hospital.All patients were assigned into control group,parecoxib single injection group and parecoxib twice injection group, respectively.Pain scaring(visual analog scale,VAS) and quality of life scoring(activity of daily living,ADL) were performed prior to and at days 1,3 and 5 after surgery in comparison with that of control group.Results: Both single and twice parecoxib injection groups exhibited significantly lower pain scales and higher quality of life scores at days 1,3 and 5 as compared with that of control group(all P<0.05 ).In addition,twice parecoxib injection group presented lower degree of pain at day 1(P<0.05),but not days 3 and 5 after surgery,as compared with that of single parecoxib injection group(both P>0.05).Furthermore,higher quality of life scores was noted in twice parecoxib injection group after at days 1 and 3(both P<0.05),but not day 5,in comparison with that of single parecoxib injection group(P<0.05).Conclusion:Parecoxib confers preferable perioperative analgesic effects for lumbar spinal stenosis,with an ideal efficacy achieved via injection for twice at day 1 and a single injection every other day thereafter.更多还原
