【摘要】 目的比较和评价骶管阻滞与骶管阻滞+坐骨神经干阻滞治疗腰椎间盘突出症的临床疗效。方法腰椎间盘突出症患者80例随机分成A、B两组,每组各40例,治疗前两组患者的性别、年龄、病变部位、病程和病情等一般资料差异无统计学意义（P>0.05）。分别行骶管阻滞和骶管阻滞+坐骨神经干阻滞治疗,两组均以曲安奈得、利多卡因、维生素B₁₂、维生素B₁与生理盐水混合液行神经阻滞治疗,每周1次,4次为一个疗程。结果经过一个疗程治疗后A、B两组的优良率分别为40%（16/40例）和88.2%（30/34例）,两组的总有效率分别为65%（26/40例）和92.5%（37/40例）,B组疗效明显高于A组（P<0.01）。A、B两组出现下肢乏力的发生率分别12.5%（5/40例）和2.5%（1/40例）（P<0.01）。结论骶管阻滞和和骶管阻滞+坐骨神经干阻滞均为治疗腰椎间盘突出症的有效方法 ,但后者比前者疗效更优,下肢乏力现象更少。更多还原

【Abstract】 Objective To compare and evaluate the clinical effects of sacral canal block(SCB) and sacral canal block with sciatic nerve block(SCB+SNB) in the treatment of lumbar intervertebral disc herniation.Methods 80 patients with lumbar intervertebral disc herniation were randomly divided into 2 groups with 40 patients in each group.The general data of sex,age,position of disease,course of disease,patient’s condition and so on for the patients in 2 groups before the treatment was not statistically significant(P>0.05).The patients were treated with SCB in group A and SCB+SNB in group B.The analgesic was composed of triamcinolone acetonide,lidocaine,vitamin B12, vitamin B1 and normal saline,which was injected into the sacral canal or sciatic nerve.This treatment in both groups was performed once a week,and 4 times was taken as a course of treatment.Results After a course of treatment, the excellent and good rates were 40%(16/40 cases) and 88.2%(30/34 cases),and the total effective rates were 65% (26/40 cases) and 92.5%(37/40 cases) respectively in group A and group B.The clinical effects of group B were obviously better than that of group A,and the differences were statistically significant(P<0.01).The occurrence rates of weakness of lower limbs were 12.5%(5/40 cases) in group A and 2.5%(1/40 cases) in group B,and the differences were statistically significant(P<0.01).Conclusions The SCB and SCB+SNB are both effective methods in the treatment of lumbar intervertebral disc herniation.The clinical effects of SCB+SNB maybe better than that of SCB,and the occurrence rate of weakness of lower limbs was lower.更多还原
