影响急性脑梗死预后的各生物学标志物联合分析

【摘要】 目的探讨各生物学标志物对急性脑梗死预后的影响。方法选取急性脑梗死病例111例,健康对照组104例,在发病24h内、发病后3d检测病例组血清S100B蛋白、髓鞘碱性蛋白(MBP)、胶质纤维酸性蛋白(GFAP)、白细胞介素-6(IL-6)浓度,在发病24h内检测血清超敏C-反应蛋白(hs-CRP)、血浆D-二聚体(DD)浓度,并于体检当天检测对照组相应标志物浓度。对各标志物浓度及风险因素进行Logistic回归分析。结果血清IL-6、S100B、吸烟、GFAP、hs-CRP、MBP、血浆DD、体质量指数、饮酒与急性脑梗死的不良结局相关,其OR值分别为3.21、2.70、2.53、2.11、1.91、1.57、1.50、1.13、1.11。结论血清IL-6、S100B、吸烟、GFAP、hs-CRP、MBP、血浆DD、体质量指数、饮酒与急性脑梗死的不良结局相关,监测其变化对急性脑梗死的预后评估有重要意义。

【Abstract】 Objective To evaluate the prognosis effects of biological markers on acute cerebral infarction.Methods 111patients with acute cerebral infarction and 104normal controls were randomly recruited.S100Bprotein,myelin basic protein(MBP),glial fibrillary acidic protein(GFAP),interleukin-6(IL-6)were determined within 24hand 3dafter acute cerebral infarction.All biological markers’concentration in normal controls were determined immediately after medical check-up.All biological markers and risk factors were evaluated by logistic regression.Results The logistic regression analysis results showed that serum IL-6,S100Bprotein,smoking,GFAP,hs-CRP,MBP,D-Dimer,body mass index,alcohol drunken were associated with poor outcomes of acute cerebral infarction,odds ratio(OR)values were 3.21,2.70,2.53,2.11,1.91,1.57,1.50,1.13and 1.11.Conclusion IL-6,S100Bprotein,smoking,glial fibrillary acidic protein,hs-CRP,myelin basic protein,D-Dimer,body mass index,alcohol drunken were associated with poor outcomes of acute cerebral infarction.It has great significance to monitor the changing of those markers for prognosis evaluation of acute cerebral infarction.
