大肠埃希菌临床分离株的分布及耐药性分析
Analysis of distribution and drug-resistance of Escherichia coli strains in clinical practice
【摘要】 目的:了解大肠埃希菌的临床分布情况及对常用抗菌药物的敏感性,为合理使用抗菌药物提供依据。方法:对我院从临床分离出的1 445株大肠埃希菌的分布及药敏结果进行回顾性分析。结果:大肠埃希菌在临床标本检出以痰液标本最常见,占32%,其余依次是分泌物(31.4%)、尿液(20.3%);大肠埃希菌对厄他培南、亚胺培南、头孢替坦的敏感性均在96%以上;1 445株大肠埃希菌中,检出ESBLs阳性780株,ESBLs检出率为54%;ESBLs阴性的大肠埃希菌对常用抗菌药物的敏感性高于ESBLs阳性者。结论:大肠埃希菌为我院呼吸道和创面感染的最常见病原菌,产ESBLs菌株的耐药性明显高于非产酶株,临床应根据耐药特点合理选用抗菌药物,防止耐药菌的产生和传播。
【Abstract】 Objective: To investigate the clinical distribution situation of Escherichia coli and the susceptibility status of commonly used antibacterial agents,aim to provide a scientific basis for rational use of antibacterial agents.Methods: The susceptibility and distribution of 1 445 strains of Escherichia coli samples from the clinical data of the Xingyi People’s Hospital were retrospectively analysed.Results: Firstly,the largest percentage of Escherichia coli strains,from clinical data,was phlegm samples,accounted 32% and secrete samples accounted 31.4%,urine samples accounted 20.3%,respectively.Secondly,in the susceptibility status aspect: specific to the Ertapenem,Imipenem and Cefotetan,the susceptibility of Escherichia coli was more than 96%.Additionally,the detection result from 1 445 strains of Escherichia coli samples showed 780 strains were ESBLs positive and also the detection rates was 54%.Finally,in the drug-resistance test,the ESBLs negative Escherichia coli samples’ susceptibility status was higher than the ESBLs positive ones.Conclusion: The Escherichia coli is the most common pathogen in the respiratory tract infection and wound infection.According to the drug-resistance characteristic:the susceptibility status of the strains which own ESBLs production capacity is higher than the ones which are non-enzyme production strains,the medical staffs should prevent the production and propagation of drug-resistance bacteria in the clinical application.
