偏髓分型及其指导应用生物型普通柄假体行人工股骨头置换治疗高龄股骨粗隆间骨折

【摘要】 目的介绍和探讨股骨粗隆间骨折偏髓分型,并分析偏髓分型指导应用生物型普通柄(短柄)假体行人工双极股骨头置换治疗高龄股骨粗隆间骨折的疗效。方法 28例高龄股骨粗隆间骨折按偏髓分型:Ⅰa型15例,Ⅰb型13例。采用生物型普通柄(短柄)假体行人工双极股骨头置换术,术后早期进行康复锻炼。结果 28例均顺利渡过围手术期,住院期间无死亡病例,术后1~3周下床活动。所有患者获6~20个月随访,平均12个月,患者均能生活自理。疗效按Harris评分标准:优13例,良12例,可2例,差1例,优良率达89.3%。结论生物型普通柄(短柄)人工股骨头置换治疗偏髓Ⅰ型高龄股骨粗隆间骨折是一种有效的治疗方法,可早期下床活动,减少并发症,提高生活质量。

【Abstract】 Objective To introduce the fracture classification of HuaTuo and investigate the clinical effect of treatment of femoral intertrochanteric fracture in the elderly patients with common handle biological artificial femoral head arthroplasty.Methods According to the fracture classification of HuaTuo,there were 15 patients with typeⅠa,13 with typeⅠb.All of 28 patients were treated with common handle biological artificial femoral head arthroplasty.The patients did earliest rehabilitation exercise after operation.Results All patients succeeded in operation and got out of the bed after 1~3 weeks.There was no death during the period of hospitalization and complications were cured.All patients were followed up for 6~20 months,with an average of 12 months.They could take care of themselves.89.3% of the patients achieved satisfactory results according to Harris rating scale.Conclusion It′ s an effective method to treat femoral interchanteric fractures in eldly patients with the common handle biological artificial femoral head prosthetic replacement.It has advantages such as earlier weight bearing,less complication,and better life quality.
