微创膝关节单髁置换术治疗70岁以上老年膝关节内侧间室骨性关节炎

【摘要】 目的探讨微创膝关节单髁置换术(MIS-UKA)治疗70岁以上老年膝关节内侧间室骨性关节炎的可行性及初步疗效。方法采用MIS-UKA治疗70岁以上老年膝关节内侧间室骨性关节炎21例(24膝)。评估切口大小、手术时间、出血量、引流量和并发症。比较术前、术后膝关节HSS评分、膝关节活动度及下肢力线(髋膝踝角)。结果术后平均随访18个月(12~24个月)。末次随访患者膝关节内侧间室疼痛明显减轻,HSS评分明显增加,下肢力线内翻畸形明显减少,但膝关节活动度无明显改善,无感染、脂肪栓塞或下肢深静脉血栓形成,假体位置不良、脱位,假体松动等并发症。结论 MIS-UKA可治疗70岁以上老年膝关节内侧间室骨性关节炎,但远期疗效尚需进一步随访。

【Abstract】 Objective To explore the feasibility and preliminary effect of minimally invasive surgery unicompartmental knee arthroplasty(MIS-UKA) for medial compartment knee osteoarthritis of patients older than 70 years.Methods All of 21 consecutive patients older than 70 years(24 knees)with medial compartment knee osteoarthritis were treated with MIS-UKA.Incision,surgical time,blood loss,drainage and complications were measured.The pre-and post-operative function of the knees were evaluated by HSS score system.The pre-and post-operative range of motion(ROM) and alignment of the lower limbs(hip-knee-ankle angle) were measured and compared.Results All cases were followed up for average 18 months(12~24 months).At the last follow up,all cases achieved significant relief of pain,improvement of HSS score and correction of varus deformity.But ROM was not significantly improved,and none had complications of infection,fat embolism,deep vein thrombosis,malposition of prosthesis,dislocation and loosing.Conclusion MIS-UKA is an effective alternative for the treatment of medial compartment knee osteoarthritis of patients older than 70 years,however,further long-term studies are required.
