圆锥型套筒冠义齿修复重度牙周炎伴牙列缺损的临床效果评价

【摘要】 目的评价圆锥型套筒冠义齿修复重度牙周炎伴牙列缺损的临床效果。方法重度牙周炎伴牙列缺损患者12例,行牙周基础治疗和根管治疗后,采用圆锥型套筒冠义齿修复,记录患者的主观感觉及义齿使用情况。修复前及修复后6个月、1年和3年复诊时,测量并记录牙龈指数(gingival index,GI)和探诊深度(probingdepth,PD),实验室检测龈沟液碱性磷酸酶(alkaline phosphatase,ALP)活性。结果 12例患者,圆锥型套筒冠义齿均可正常使用,9例患者对外貌及义齿的使用和语音功能满意,3例基本满意,无不满意。牙龈指数GI在修复后6个月、1年、3年和修复前相比差异无统计学意义(P>0.05)。PD在修复后6个月、1年、3年和修复前相比差异均有统计学意义(P<0.01)。龈沟液ALP活性在修复后6个月和修复前相比差异均有统计学意义(P<0.01),而在修复后1年、3年和修复前相比差异亦有统计学意义(P<0.05)。结论对于重度牙周炎伴牙列缺损,圆锥型套筒冠义齿是一种可行、有效的修复方法,同时也有助于患者牙周状况的改善。

【Abstract】 Objective To evaluate the clinical effect of telescopic prostheses on restoring severe periodontitis combined with dental arch defect.Methods Twelve patients with severe periodontitis and dental arch defect were restored with telescopic prostheses after non-operative treatment and root canal therapy.Patients’ evaluation of dentures as well as gingival index(GI),probing depth(PD),and alkaline phosphatase(ALP) activity of gingival cervical fluid were recorded.Results After 3 years,all the dentures functioned well.Nine patients were very satisfied with appearance and phonetic functions,and the rest 3 patients were basically satisfied.Compared to baseline,there was no significant change from 6 months to 3 years(P>0.05).On the other hand,the PD and ALP activity changed significantly from 6 months to 3 years(P<0.05).Conclusion For patients with severe periodontitis and dental arch defect,telescopic prosthese is a feasible and effective method especially in improving periodontal health.
