【摘要】 目的:总结自膨式支架植入术治疗非出血性椎基底动脉夹层的经验。方法:回顾性分析16例非出血性椎基底动脉夹层的临床特点、影像表现和支架植入方式以及随访结果。结果:所有病例支架均成功植入,围手术期无严重的并发症。8例有头痛表现的患者症状均明显好转。1例患者单支架植入后2年停用抗血小板药物3d支架内血栓形成,急诊动脉溶栓后再通。其余所有病例支架内血流均通畅,无脑出血以及与夹层相关的缺血性脑卒中。7例双支架植入的病例夹层均明显减退或消失。结论:选择合适的病例和支架植入方式,自膨式支架植入是非出血性椎基底动脉夹层安全、有效的治疗方法。双支架技术有良好的临床应用远景。更多还原

【Abstract】 Objective:To summarize the experience of self-expanding stent implantation for the treatment of non-hemorrhagic vertebrobasilar dissection.Methods:The clinical features,image findings,stent implantation strategies and follow-up results of 16 cases of non-hemorrhagic vertebrobasilar dissection were retrospectively analyzed.Results:All the stent implantations were successfully performed with no severe complications during the perioperative period.The headache in 8 patients was relieved obviously.In 1 case two years after single stent implantion,when the antiplatelet agent was withdrawn for three days,thrombosis developed in the stent.It was removed by intra-arterial thrombolysis in emergency.In all other cases,the blood flow in the stent was in good condition and there was no hemorrhage or any dissection related stroke accident.The dissection was degraded or disappeared obviously in all 7 cases treated with overlapped double stent implantation.Conclusion:With selective patients and proper stent implantation strategy,the self-expanding stent implantation is a technically effective and safe method for the treatment of non-hemorrhagic vertebrobasilar dissection.The overlapped double stenting technique has a good outlook in clinical applications.更多还原
