【摘要】 目的探讨电视胸腔镜手术(VATS)对肺部结节病灶的诊治价值。方法对102例有肺部结节病灶的患者,运用电视胸腔镜行结节所在肺组织的楔形切除,术中送冰冻切片病理检查。结果 102例患者中良性病变83例,其中炎性假瘤36例,结核瘤26例,肺囊肿2例,错构瘤12例,肺曲菌病4例,肺隐球菌病3例;19例为肺癌,其中细支气管肺泡癌8例,鳞癌6例,腺癌3例,转移性肿瘤2例。83例良性病变均行VATS单纯肺楔形切除,16例肺癌行辅助小切口肺叶切除加淋巴结清扫,3例肺癌患者因混合性通气功能障碍行VATS下楔形切除术。全组患者无围手术期死亡,未出现严重的手术并发症。结论 VATS作为一种新手段,在肺部结节的诊治方面具有诊断与治疗双重功能,且具有确诊率高,创伤小等优点。更多还原

【Abstract】 Objective To sum up the experience and comprehending of video-assisted thoracoscopic surgery(VATS)in the diagnosis of pulmonary nodules.Methods A total of 102 patients with pulmonary nodules were treated by pulmonary wedge resection through video-assisted thoracoscopic surgery(VATS).During the surgery pathological findings of intraoperatice frozen section biopsy was performed.Results There were 102 patients,19 were lung cancer(three was adenocarcinoma,eight were bronchinloalveolar carcinoma,6 were squamous cell carcinoma,2 was metastatic tumor),36 were inflammation inflammatory pseudotumour,26 were tuburculoma,12 were hamartoma,2 were lung cyst,4 was lung aspergillomycosis,three was Pulmonary cryptococcosis.Amongho 2 patients,83 patients underwent pulmonary wedge resection through VATS,16 patients accepted a radical resection of the tumor and a meditational lymph node resection and 3 patients was performed a palliative wedge resection.All the operations were successfully completed and no complications or deaths occurred during latest follow-up.Conclusion This study confirmed that VATS has high diagnostic rate with small wound,and rapid postoperative recovery.更多还原
