孤立性肺结核瘤误诊3例CT影像学分析

【摘要】 目的分析肺结核瘤的CT影像学表现特点,提高其鉴别认识。方法收集经手术病理确诊的3例肺结核瘤术前误诊为肺癌的病例进行分析。结果 3例肺结核瘤的主要CT表现为其直径均小于40 mm,分别为35 mm、33mm、27 mm,边缘均不规整,可见长短不一的棘状突起、毛刺;2例有浅分叶,1例局部凹陷;病灶内无钙化,2例无空洞,1例可见小空洞;3例病灶周围无卫星灶;增强后病灶有不同程度的强化、且病灶周边部分强化明显;纵膈内可见肿大的淋巴结。结论 CT对结核瘤的检出具有较高的价值,认真全面的分析其影像特点,有助于结核瘤的判断;但对于孤立的、不典型的结核瘤还是常常与肺肿瘤不易鉴别,最终结论还有待于病理诊断。

【Abstract】 Objective To analyze the characteristics of CT for solitary pulmonary tuberculoma and improve identification.Methods Three cases of pulmonary tuberculoma diagnosed by pathobiology and misdiagnosed as carcinoma of the lungs were analyzed and discussed with reference to relevant documents.Results he primary imaging appearances of the three cases on CT are less than 40mm in diameter,namely,35 mm,33 mm,27 mm.The tumor edge is not structured with spinous process,spiculation sign;two cases showed shallow lobulation,one case with local denting sign;three cases showed satellite nodule;the lesion was enhanced to different degrees,the perifocal area was obviously enhanced and visual mediastinal enlarged lymph nodes.Conclusion CT has highly diagnostic value for pulmonary tuberculoma.The comprehensive analysis of the imaging characteristics is helpful to distinguish tuberculoma;but it is still difficult to discriminate solitary,atypical tuberculoma between lung tumors.The final result depends on pathobiology.
