舟山海岛成人碘营养状况调查与分析
Investigation and analysis on iodine nutrition of adults in Zhoushan islands
【摘要】 目的了解舟山海岛成人碘营养状况,为制定和完善科学补碘策略提供依据。方法抽取舟山市2个县、区中6个居委会(村)3 098名当地18岁以上的常住居民,共采集1 683份碘盐、20份水样、2 872份尿样分别进行盐碘、水碘、尿碘浓度检测,并对调查对象开展甲状腺超声检查,采集血样进行甲状腺功能检测。结果盐碘浓度的中位数为0 mg/kg、均数5.63 mg/kg;有283份为合格碘盐,合格碘盐食用率为16.82%;水碘浓度均数为11.40μg/L;尿碘中位数为112.50μg/L;甲状腺超声检查总异常率30.63%;甲状腺功能总异常率为21.98%。结论舟山市成人居民的尿碘水平处于较适宜的水平;成人整体甲状腺超声检查异常率高于全省水平,18~65岁成人甲状腺超声检查异常率明显低于全省水平,成人女性甲状腺超声检查异常率明显高于男性,65岁以上老年人甲状腺结节发生率为38.49%,明显高于全省水平;甲状腺功能激素异常率低于全省水平。
【Abstract】 Objective To access the iodine nutrition of adults in Zhoushan islands and provide references for developing and improving scientific strategy of iodine supplementation.Method 3 098 adults over 18 years old who were local permanent residents in six communities(villages) of two districts were sampled randomly.1 683 iodized salt samples,20 drinking water samples and 2 872 urine samples were tested respectively to measure their concentrations.The thyroids of sample were checked by ultrasonography and blood samples were tested for thyroid function accessment.Result The median salt iodine concentration was 0 mg/kg and mean was 5.63 mg/kg.Only 283 salt samples were adequately iodized salt with a qualified rate of 16.82%.The medians of water iodine and urinary iodine concentration were 11.40 μg/L and 112.50 μg/L respectively.The total goiter rate was 30.63% and the total thyroid dysfunction rate was 21.98%.Conclusion The level of urinary iodine of adults in Zhoushan city was moderate.The total goiter rate of adults was significantly higher and the total goiter rate of adults aged 18 to 65 years old was significantly lower than average level of Zhejiang province.The total goiter rate of female adults was significantly higher than that of male.The incidence of thyroid nodule in eldly over 65 was 38.49%,which was significantly higher than average level of Zhejiang province.The total thyroid dysfunction rate was lower than average level of Zhejiang province.
