CT三维重建对肝胆管结石成因分析及手术抉择的应用价值研究

【摘要】 目的研究CT三维重建技术对肝胆管结石成因分析及手术方式选择的临床应用价值。方法以2011年1月~2011年7月本科室20例复杂肝胆管结石患者为研究对象,采集肝胆管结石患者64排螺旋CT扫描二维数据集,运用自行开发的软件-医学图像处理系统Medical Image Process System(MIPS)进行自动程序分割,将肝脏和各管道模型以STL(StandardTemplate Library)格式导入FreeForm Modeling System进行三维重建;通过对三维模型的全方位,多角度,多层次观察,分析确定各部位肝内胆管结石的成因,并据此制定合理手术方案。结果三维重建所获得的肝胆模型结构清晰、立体感强,形态逼真,交互性好;通过将肝脏及胆道系统透明化处理,可直观分析各部位肝胆管结石与周围组织结构的空间位置关系,为结石成因分析及手术方式确定提供可靠依据。结论 CT三维重建技术对肝胆管结石成因分析及手术方式的选择具有良好的应用价值。

【Abstract】 Objective To study the value of three - dimentional reconstruction(3D) technique to cases with hepatolithiasis on analysis of stone formation and operation plan based on CT scanning data.Methods We collect 3 patients randomly from 20 cases with hepatolithiasis admitted from January 2010 to July 2011,use the self- developmented software MIPS(Medical Image Process System) to make the automatic image sequance segmentation and 3D reconstruction based on their 64 - row helical CT scanning data,import their 3D models to the FreeForm Modeling System with STL formatting,and then analysis the reason of stone formation and make the reasonable operation plans by researching the 3D models from all directions,multiple angles,and multi - strata.Results 3D models provide clarity with strong relief perception and a user - friendly interface,we can get accurate digital information about stone formation,and choose the best surgical plan according to it.Conclusion 3D technique based on CT scanning is more objective and complete than routine preoperational examinations to choose the best operation plan for intrahepatic calculi.
