【摘要】 目的:研究ω-3多不饱和脂肪酸(PUFA)对胃癌病人术后炎性反应和疲劳综合征的影响。方法:42例胃癌病人随机分为对照组(n=23)和试验组(n=19)。对照组病人按低氮、低热量原则,于术后连续5 d行肠外营养(PN)治疗,试验组病人加用ω-3 PUFA。检测两组病人术前和术后第1、3、5天炎症因子IL-1β、IL-2、IL-6、TNF-α、IL-12水平和术前及术后第1、5天肝肾功能,同时记录病人术前和术后第1、3、5、7天的疲劳评分、术后肛门排气时间和并发症的发生情况。结果:术后第3天IL-1β、TNF-α和IL-6浓度最高,两组无显著性差异。术后第5天,试验组IL-1-β、TNF-α和IL-6浓度明显低于对照组(P<0.05)。疲劳评分分值术后第3天最高,试验组病人术后第3、5、7天疲劳评分均低于对照组,两组差异有统计学意义(P<0.05)。两组病人GPT、GOT、肌酐和尿酸术后第5天较第1天均有下降,试验组下降更明显,与对照组比有显著性差异(P<0.05)。结论:ω-3 PUFA能减轻胃癌病人术后的炎性反应和疲劳,促进术后康复。更多还原

【Abstract】 Objective: To investigate the effect of omega-3 polyunsaturated fatty acid on inflammatory reaction and postoperative fatigue of patients with gastric carcinoma after operation.Methods: 42 cases with gastric carcinoma were randomly divided into control group(n=23) and study group(n=19).The two group patients were treated with hypocaloric parenteral nutrition,and the study group received ω-3 fatty acids additionally.The total parenteral nutrition was transfused from the first day to the fifth day after operation.The levels of serum IL-1-β,IL-2,IL-6,TNF-α,IL-12 were detected on the day before the operation,the first,third and fifth day after operation.Liver function,kidney function were detected on the day before the operation,the first and fifth day after operation.Fatigue score were measured on day before the operation,the first,third,fifth and seventh day after operation.Results:IL-1-β,TNF-α,IL-6 in the study group were significantly lower than the control group on postoperative day 5.The study group had lower fatigue score on postoperative days 3,5,7.ALT,AST,creatinine and uric acid in two group were decreased on postoperative day 5 and the study group have a obvious drop(P <0.05).Conclusion: Early total parenteral nutrition contained ω-3 polyunsaturated fatty acids can restrain inflammatory reaction,alleviate postoperative fatigue.更多还原
