螺旋CT三维重建技术对股骨后髁角个体化测量的应用研究
Application of Spiral CT Three-dimensional Reconstruction Technique on Individual Measurement of Femoral Posterior Condylar
【摘要】 目的探讨股骨后髁角(PCA)个体化测量的方法及可行性,为全膝关节置换术(TKA)股骨髁外旋截骨提供个体化的参考角度。方法选择50例正常成人(对照组)及50例TKA患者手术前(实验组)膝关节的CT资料,由两位观测者分别测量PCA各3次取平均值,比较不同观测者之间测量数据的差异,比较实验组与对照组组间的差异,P<0.05认为差异具有统计学意义。结果对不同观测者测量所得的同组PCA均数进行比较,差异无统计学意义(P>0.05);对照组的PCA值为3.64°±0.89°,实验组PCA值为4.23°±1.48°,实验组与对照组比较,差异有统计学意义(P<0.05)。结论 PCA测量结果准确,恒定,误差小,可重复性强。在TKA患者手术前,首先应对PCA进行准确的个体化测量,并将其作股骨髁旋转截骨的参考角度,为临床指导手术截骨、正确进行旋转对线提供测量依据。
【Abstract】 Objective To investigate methods and feasibility of individual measurement of posterior condylar angle(PCA),to provide reference for total knee arthroplasty(TKA) femoral condyle external rotation osteotomy.Methods CT data of the knee of 50 normal adults(control group) and 50 cases with TKA before operation(experimental group) were collected.PCA was measured by 2 observes 3 times,and the average was calculated.The differences of the data between observers and the differences between the experimental group and the control group were compared.The differences had statistical significance if P <0.05.Results The differences of the average of femoral PCA between two observers in the same group was no statistical significance(P > 0.05);The PCA value of the control group and the experimental group were 3.64 °± 0.89 °and 4.23 ° ± 1.48 ° respectively.The differences between the two groups was statistically significant(P <0.05).Conclusion The measurements of PCA are accurate,constant,and the error is small and have strong repeatability.Before operation,patients with TKA should undergo individual measurement of PCA as the reference for femoral condyle rotational osteotomy,and provide measurement basis for osteotomy and accurate rotational alignment.
