儿童小脑幕下区的磁共振灌注成像技术研究
The Study of MR PWI in Infratentorial Region of Children Brain
【摘要】 目的研究磁共振灌注成像(MRPWI)技术应用于儿童小脑幕下区血流灌注分析的可行性及其方法。方法研究MRPWI检查患者86例,其中实验A组采用单通道MRI扫描仪检查患儿22例、成人患者16例,取听眦线扫描;实验B组采用8通道MRI扫描仪检查患儿28例、成人患者13例,取听眦线扫描;实验C组采用8通道MRI扫描仪检查患儿7例,取听上眶线扫描。MRPWI图像资料处理得到负性增强积分(NEI)伪彩图,以NEI伪彩图为参考图像,选取经第四脑室层面图像,在乙状窦内侧旁与第四脑室外侧旁脑实质之间取一最长连线,在该线段上由乙状窦内侧旁脑实质至第四脑室外侧旁脑实质等距取3个ROI(依次定义为1~3区)作为研究区域,通过3个区域的MRPWI信号-时间曲线分析,以评估幕下区MRPWI有效性及伪影严重性。结果儿童小脑幕下区MRPWI图像伪影分级评估分析结果表明8通道线圈实验组优于单通道线圈实验组、成人组优于儿童组、听上眶线组优于听眦线组,其是否有效取决于ROI-2区。结论儿童小脑幕下区MRPWI血流灌注分析前必须先做小脑幕下区MRPWI图像伪影分级评估以确定其有效性,采用更高级别的MRI扫描仪并取听上眶线扫描均有利于提高儿童小脑幕下区MRPWI有效灌注率。
【Abstract】 Objective To study the feasibility and method of Magnetic Resonance Perfusion Weighted Imaging (MR PWI) in infratentorial region of children brain. Methods Patients (n=86) undergone MR PWI were divided into 3 groups. In group A, 22 children and 16 adults were scanned by single channel MRI on Orbitomeatal Basal Line (OMBL). In group B, 28 children and 13 adults were scanned by 8-channel MRI on OMBL. In group C, 7 children were scanned by 8-channel MRI on Glabellomeata Line(GML). Equidistant 3 regions of interest (ROI 1~3) were set from the cerebel near sinussigmoideus to the one near the fourthventricle. The degree of artifact and MR PWI availability in infratentorial region were analyzed according to the time-signal curves of 3 ROIs. Results The study of infratentorial region artifact grading in MR PWI show that 8-channel MRI scanner was better than single channel one, adults were better than children and GML scan was better than OMBL. Conclusion Artifact grading must do first before the study of MR PWI in infratentorial region, and high class MRI scanner with GML scan would do good for the children patients.
