冠脉内应用替罗非班对急诊PCI术患者血小板活化功能的影响

【摘要】 目的探讨急性ST段抬高型心肌梗死患者急诊直接经皮冠状动脉介入治疗(PCI)术中冠脉内应用替罗非班对患者血小板活化功能的影响。方法 2010年12月—2011年11月,共70例接受急诊直接PCI治疗的急性ST段抬高型心肌梗死患者纳入研究,其中男性49例,女性21例,随机分为替罗非班组和对照组,两组各35例。两组患者均于发病12 h内行急诊PCI术,替罗非班组:术中支架植入前冠状动脉内注入替罗非班体质量10μg/kg;对照组:术中直接行支架术。观察两组患者术中血小板活化标志血小板a-颗粒膜蛋白(GMP140)水平。结果替罗非班组血浆a颗粒膜蛋白浓度(17.99±1.01)μg/L低于对照组(24.56±1.96)μg/L,两组相比较有统计学差异(P<0.01)。结论研究表明,急性ST段抬高型心肌梗死患者急诊PCI术中联合冠脉内应用替罗非班可以抑制血小板活化功能。

【Abstract】 Objective To investigate the influence of intracoronary administration of tirofiban bolus in patients with acute ST segment elevation myocardial infarction(STEMI) undergoing emergency percutaneous coronary intervention(PCI) on platelet activation.Methods Fifty patients with acute myocardial infarction who underwent emergency PCI within 12 hours were randomly divided into 2 groups due to the random number table method:tirofiban group(35 patients)and control group(35 patients);male(49) and female(21).The tirofiban group additionally received intracoronary tirofiban hydrochloride injection of 10 μg/kg before bracket implantation during PCI.The control group received bracket implantation directly.To observe the changes of platelet activation(platelet a grains membrane protein) during PC.Results The GMP140 concentrations of tirofiban group(17.99±1.01) μg /L were lower than control group(24.56±1.96) μg /L;there was statistics difference between the 2 groups(P<0.01).Conclusion Intracoronary administration of tirofiban bolus in patients with STEMI undergoing acute PCI can inhibit platelet activation.
