肠炎清Ⅰ号合锡类散保留灌肠治疗湿热内蕴型溃疡性结肠炎15例
Changyanqing Ⅰ Combined with Xilei Powder in Treatment of Ulcerative Colitis of Dampness and Heat Type:A Report of 15 Cases
【摘要】 目的观察肠炎清Ⅰ号保留灌肠治疗湿热内蕴型溃疡性结肠炎(ulcerative colitis,UC)的临床疗效及其对血清IL-8和IL-4影响。方法将30例湿热内蕴型UC患者,随机分为治疗组与对照组,每组各15例,治疗组用肠炎清Ⅰ号加锡类散0.75g,常规保留灌肠。对照组用锡类散0.75g,保留灌肠。两组患者均每日口服奥沙拉秦3g。临床观察两组治疗前后疾病活动指数(disease activity index,DAI)、电子结肠镜下黏膜病理变化、血清白介素-8(interleukin-8,IL-8)和白介素-4(interleukin-4,IL-4)含量变化。结果治疗后治疗组DAI显著低于对照组(P<0.01);电子结肠镜显示治疗组黏膜炎症的改善程度显著优于对照组(P<0.05);治疗组血清IL-8水平的下降和IL-4水平的升高显著优于对照组(P<0.05,或P<0.01)。结论肠炎清Ⅰ号合锡类散灌肠治疗UC疗效显著,下调IL-8水平和升高IL-4水平可能是其治疗UC的作用机制之一。
【Abstract】 Objective To observe the effect of Changyanqing Ⅰ retention enema in treating ulcerative colitis(UC) of dampness and heat type,and observe its effect on the serum levels of interleukin-8(IL-8) and interleukin-4(IL-4).Methods Thirty UC patients with syndrome type of dampness and heat were averagely randomized into a treatment group and a control group.Patients in the treatment group were treated with conventional retention enema of Changyanqing Ⅰ and Xilei Powder,while patients in the controlgroup were treated with retention enema of Xilei Powder only.Patients in both groups were treated with Olsalazine.The disease activity index(DAI),pathological changes of mucosa under electronic colonoscopy,and the content changes of IL-8 and IL-4 were observed.Results After treatment,DAI in the treatment group was significantly lower than that in the control group(P<0.01).Electronic colonoscopy showed that the improvement of mucosal inflammation in the treatment group was superior to that in the control group(P<0.05).The level of IL-8 was significantly lower than that in the control group(P<0.01),while the level of IL-4 was significantly higher than that in the control group(P<0.05).Conclusion Changyanqing Ⅰ combined with Xilei Powder has significant effect in treating UC.Decreasing the level of IL-8 and increasing the level of IL-4 may be one of the mechanisms of treating UC.
