【摘要】 目的探讨经尿道前列腺电切术(TURP)联合钬激光碎石同期处理前列腺增生合并膀胱结石的疗效和优点。方法回顾TURP联合钬激光碎石同期处理前列腺增生合并膀胱结石80例临床资料,分析手术时间、术中出血、术后症状改善情况等。结果 80例前列腺增生合并膀胱结石均一次手术成功,手术时间为35～110 min,平均62 min。术中出血量为20～100 ml,平均60 ml,无大出血及输血病例。术后无结石残留,术后2～6 d拔除尿管,均能自行排尿,无尿失禁。术后1个月时IPSS评分下降至5～20分,平均12分;MRF上升到13～25 ml/s,平均16.5 ml/s;PVR为0～40 ml,平均15 ml。手术前后比较差异均有统计学意义(P<0.01)。结论 TURP联合钬激光碎石同期处理前列腺增生合并膀胱结石是一种安全有效的方法,具有创伤小、术后恢复快等优点,值得临床进一步应用及推广。更多还原

【Abstract】 Objective To explore the efficacy of TURP with Ho.YAG laser lithotripsy for the treatment of benign prostatic hyperplasia incorporated with vesical calculus.Methods The clinical data of 80 cases benign prostatic hyperplasia incorporated with vesical calculus by TURP with Ho.YAG laser lithotripsy were reviewed and the operation time,vessel bleeding,the improvement of postoperative symptom,etc were analyzed.Results The surgeries of 80 cases benign prostatic hyperplasia combined with bladder stone were all successful with only one time.The operation time was 35-100 min,and the average was 62 min.The vessel bleeding was 20~110 ml,and the average was 60 ml,with no cases of hemorrhage and blood transfusion.No residual stones were found after operation.2-6 d removal of urinary catheter after operation,all patients were able to urinate on their own,without urinary incontinence.One month after operation,IPSS score dropped to 5-20,averaging 12 points;MRF rose to 13~25 ml/s,with an average of 16.5 ml/s;PVR was for 0~40 ml,with an average of 15 ml.The difference was statistically significant compared before and after surgery(P<0.01).Conclusion TURP combined with Ho.YAG laser lithotripsy for treatment of BPH with bladder stones in the same period is a safe and effective way with small wounds and quick postoperative recovery.It is worth the further clinical application and promotion.更多还原
