肿瘤筛查中3T MR背景抑制全身弥散成像技术探讨
The technical application of whole body diffusion weighted imaging with background suppression(DWIBS) of 3TMR in screening of oncology
【摘要】 目的探讨肿瘤筛查中3T MR背景抑制全身弥散成像(DWIBS)最佳技术。方法使用西门子Verio3.0T超导型MR扫描仪,15例志愿者及10例肿瘤患者行DWIBS检查。调节不同参数b值、NEX、呼吸方式、背景抑制方式进行对比扫描,以ADC值、质量指数、SNR为指标统计分析;对原始图像进行3DMIP、黑白翻转等后处理。结果 (1)随着b值增大,图像的ADC值、SNR、质量指数均变小;b值=800能获得良好效果;(2)随NEX增大,图像的SNR、质量指数差异有统计学意义。(3)呼吸门控较平静呼吸SNR、质量指数差异较小;(4)STIR较SPAIR质量指数高;(5)厚块全身MIP、黑白反转等后处理能敏感显示肿瘤及全身转移;薄层MIP对病灶的解剖结构显示好。结论选取b值=800、NEX=4、平静呼吸、STIR背景抑制能获得高质量的DWIBS图像;通过后处理技术能达到类"PET"效果。
【Abstract】 Objective To explore the best technique of the whole body diffusion weighted imaging with background suppression(DWIBS) of 3TMR in screening of oncology.Methods DWIBS with 3TMR from Siemens was performed on 15 healthy volunteers and 10 patients with tumor,by adjusting the differen parameter values of b,TR,NEX,respiratory way respectively; The value of ADC,quality index and SNR as the indicators for statistical analysis.DWIBS primary images with 3DMIP reconstruction and black--white inersion were compared.Results(1) when the augmentation of b value,the value of ADC,quality index and SNR were dimin,but b value as 800,the image quality is better.(2) With the augmentation of NEX,there are significant difference in SNR and the quality index.(3) There are small differences in SNR and the quality index between respiratory gating and still breathing image.(4) The quality index of STIR is higher than the SPAIR’s.(5) 3DMIP reconstruction and black -white inersion can be sensitive to show tumor and all over the body metastasis.Thin layer MIP can show the lesions’ anatomical structure.Conclusion As b = 800,NEX = 4,respiratory gating,STIR with background suppression can get high quality image of DWIBS.The postprocessing imaging can achieve similar PET image(DWIBS primary images with 3DMIP reconstruction and black--white inersion).
