【摘要】 目的探讨癫痫痫样放电与睡眠时相的关系及癫痫对睡眠结构的影响。方法对85例癫痫患者的动态脑电图(AEEG)监测,并对结果进行分析。结果 85例癫痫患者中痫样放电共61例(71.8%),睡眠期出现56例(91.8%),以NREM Ⅰ、Ⅱ期最多见。睡眠中全身性发作41例、部分性发作继发全身性发作4例均出现于NREMⅠ、Ⅱ期。与对照组比较,癫痫患者总睡眠时间、REM期较正常对照组差异无显著性(P>0.05),但NREMⅠ、Ⅱ期显著延长,而NREMⅢ、Ⅳ期明显缩短,睡眠潜伏期延长(P<0.01)。癫痫患者睡眠纺锤波出现不对称、减少或消失。结论痫样放电主要发生于睡眠期NREMⅠ、Ⅱ期,癫痫也改变着睡眠结构,引起睡眠障碍。不同癫痫发作类型与睡眠时相有一定的关系。更多还原

【Abstract】 【Objective】 To explore the relationship between epileptiform discharges and sleep phase and the influence of epilepsy on sleep construction.【Methods】 AEEG monitoring was performed in 85 ones and the results was analyzed.【Results】 Epileptiform discharges were observed in 61 patients(71.8%)of 85 ones.Epileptiform discharges emerged in 56 patients(91.8%) during sleep and was mainly found in the Ⅰ～Ⅱstages of NREM sleep.41 of generalized seizures and 4 of partial seizures lead to generalized seizures all happened during the Ⅰ～Ⅱstages of NREM sleep.There was no difference between the total sleep duration,the stage of REM and the control group(P >0.05).But the durations of Ⅰ～Ⅱstages of NREM sleep were notably prolonged,and that of the Ⅲ～Ⅳstages of NREM sleep shortened.The sleep concealed durations was prolonged(P <0.01).Sleep spindles were symmetrical decreased and vanished.【Conclusions】 Epileptiform discharges mainly happened during Ⅰ～Ⅱstages of NREM sleep.Epilepsy also changed sleep construction and led to sleep obstruction.Different types of epilepsy have specific relation to sleep phases.更多还原
