恶性肿瘤住院患者合并铜绿假单胞菌感染及耐药性分析

【摘要】 目的:探讨我院恶性肿瘤住院患者合并铜绿假单胞菌(PAE)感染的耐药情况,为临床合理用药提供科学依据。方法:细菌鉴定采用手工法和美国MicroScan WalkAway40SI全自动微生物分析仪进行。药敏试验采用K-B纸片扩散法,用WHONET5.4软件进行统计分析。结果:本研究共检出G-杆菌2889株,其中PAE 254株(8.8%);PAE感染有逐年上升趋势(P<0.05)。PAE感染主要集中于内科、放疗和ICU等科室;PAE对环丙沙星的耐药率最低(8.3%)、其次为头孢哌酮/舒巴坦(10.2%)、头孢他啶(11.0%)、阿米卡星(11.4%)、美洛培南(11.8%)、哌拉西林/他唑巴坦(13.4%)、亚胺培南(14.2%)、头孢吡肟(14.2%)、左氧氟沙星(14.6%)等,而对复方新诺明的耐药率最高(95.3%)。结论:PAE是我院恶性肿瘤住院患者合并感染的主要病原菌之一,对多种抗菌药物均产生不同程度的耐药性。

【Abstract】 Objective:To provide scientific basis for clinical rational drug use in our hospital through exploration on drug resistance of inpatients with malignant tumors complicated with pseudomonas aeruginosa infection.Methods:Identification of bacteria was realized by using manual method and the U.S.MicroScan WalkAway40SI automatic microorganism analyzer.Drug sensitivity test was done by K-B disk diffusion method with WHONET5.4 software for statistical analysis.Results:2889 strains were detected with G-bacillus,including PAE strain 254(8.8%);PAE infection is increasing year by year(P<0.05).The PAE infection mainly focus on internal medicine wards and ICU departments in radiotherapy ward;PAE on ciprofloxacin resistance rate was the lowest(8.3%),followed by cefoperazone/shu ba stein(10.2%),ceftazidime(11%),amikacin(11.4%),meropenem(11.8%) and piperacillin/he zuo batan(13.4%),imipenem(14.2%),cefepime(14.2%),levofloxacin(14.6%),and resistance rate to cotrimoxazole was the highest(95.3%).Conclusion:PAE is one of the main pathogens in inpatients with malignant tumors complicated with infection in our hospital,which has varying degrees of resistance to different kinds of antimicrobial agents.
