CBA技术检测胃肠癌患者Th/1Th2细胞因子表达水平的研究
Investigation in the levels of Th1 and Th2 type cytokines in patients with gastric and colorectal cancer by CBA
【摘要】 目的:探讨胃肠癌患者血清Th1/Th2种细胞因子变化水平及其临床意义,为消化道肿瘤免疫治疗提供依据。方法:采用流式细胞小球微阵列技术(cytometric bead array,CBA)检测100例胃肠癌患者及70例健康人血清中γ干扰素(IFN-γ)、肿瘤坏死因子(TNF-α)、白细胞介素(IL-2、IL-4、IL-5、IL-10)表达水平。结果:100例胃肠癌患者血清Th1细胞因子IFN-γ、IL-2含量分别为19.85±5.36 pg/ml、3.50±1.29 pg/ml,Th2细胞因子IL-4、IL-5含量为4.60±1.52 pg/ml、2.83±0.73 pg/ml均明显高于健康体检组(P<0.01);肿瘤组TNF-α含量为4.35±3.53 pg/ml低于健康体检组(P<0.01);胃肠癌组IL-2/IL-4比值降低(0.79±0.49),与健康体检组相比差别有统计学意义(P<0.01)。IL-4水平与肿瘤分化程度有关,低分化癌组IL-4水平(4.85±1.25 pg/ml)高于高-中分化癌组(4.02±1.42 pg/ml)(P<0.05);IL-4与疾病进展相关,IV期患者血IL-4水平(4.90±1.08 pg/ml)高于其他各期(I期3.80±1.45 pg/ml、II期4.10±1.79 pg/ml、III期4.70±1.59 pg/ml)(P<0.05)。结论:胃肠道癌患者体内Th1和Th2类细胞因子表达水平紊乱,IL-4表达水平与肿瘤临床分期、分化程度相关,检测胃肠道癌患者血清中Th1/Th2细胞因子的表达可作为评价肿瘤临床进展及预后指标。
【Abstract】 Objective:To detect the levels of Th1 and Th2 type cytokines in the patients with gastric and colorectal cancer in order to find out the laboratory evidence of tumor immunotherapy.Methods: The levels of serum IFN-γ,TNF-α,IL-2,IL-4,IL-5,IL-10 were measured by cytometric bead array in 100 patients with gastric and colorectal cancer,and compared with those in control of whom 70 persons were for health examination.Results: The levels of Th1 type cytokines in 100 patients with gastric and colorectal cancer were: IFN-γ(19.85±5.36 pg/ml),IL-2(3.50±1.29 pg/ml),TNF-α(4.35±3.53 pg/ml),and the levels of Th2 type cytokines were IL-4(4.60±1.52 pg/ml),IL-5(2.83±0.73 pg/ml) and IL-10(3.30±1.33 pg/ml).The levels of both Th1 and Th2 cytokines were significantly higher those in controls(P<0.01) except TNF-α and IL-10.The IL-2/IL-4 ratio(0.79±0.49) was significantly decreased in patients with gastric and colorectal cancer.The level of IL-4 was higher in patients with poorly-differentiated(4.85±1.25 pg/ml) compared with those in the well-moderately differentiated(4.02±1.42 pg/ml)(P<0.05),and stage IV(4.90±1.08 pg/ml)compared with those in the stage I(3.80±1.45 pg/ml)or stage II(4.10±1.79 pg/ml) or stage III(4.70±1.59 pg/ml)(P<0.05).Conclusion: The levels of both Th1 and Th2 cytokines were disequilibrium in patients with gastric and colorectal cancer.The expression of IL-4 was higher in advanced cases.It is valuable for clinical evaluation of progression and prognosis of gastric and colorectal cancer that combined detection of 6 serum cytokines by cytometric bead array.
