良恶性椎体压缩性骨折的MRI诊断价值
MRI Diagnostic Value of Benign and Malignant Vertebral Compression Fractures
【摘要】 目的:探讨椎体压缩性骨折的MRI表现及诊断价值。方法:回顾性分析总结了113例良恶性椎体压缩性骨折的MRI表现,其中良性67例201个病变椎体,恶性46例138个病变椎体。结果:恶性压缩性骨折46例138个病变椎体中,T1WI表现为均匀低信号的病例占89%,病变位于椎体后2/3的病例占85%,椎弓根破坏占76%,椎体后缘外凸占78%,倒楔形椎体形态改变占43%。良性压缩性骨折中67例201个压缩椎体骨折中,骨折椎体T1WI信号不均匀或者信号改变不明显占76%,椎体楔形改变占73%,压缩骨折的椎体连续性分布占45%,椎间盘信号不均匀占51%,病变位于椎体前1/3的病例占48%。结论:椎体良恶性压缩骨折的MRI表现存在差异,MRI检查可以区别椎体良恶性压缩骨折。
【Abstract】 Objective: To investigate the different features and diagnostic value of MRI between benign and malignant vertebral compression fractures.Methods: 113 cases of MRI features of benign and malig-nant vertebral compression fractures were summarized retrospectively.There are 201 vertebrae lesions of 67 cases with benign compression fractures,and 138 vertebrae lesions of 46 cases with malignant com-pression fractures.Results: All of 138 vertebra lesions of 46 cases with malignant compression fractures,T1WI showed homogeneous low signal 89%,the lesions in the posterior 2/3 of the vertebra 85%,de-struction of the pedicle 76%,the convex lesions in vertebral posterior margin 78%,inverted wedge-shaped vertebral changes 43%.All of 201 vertebra lesions of 67 cases with benign compression fractures,76% cases T1WI vertebral signal changes is not obvious same as normal,vertebral wedge change 73%,vertebrae continuous distribution of compression fractures accounted for 45%,disc signal nonuniform 51%,lesions in prior to 1/3 of the vertebral body accounted for 48%.Conclusion: Benign and malig-nant vertebral compression fractures show significant differences in MRI,MRI examination can distingu-ish benign and malignant vertebral compression fractures.
