SPA金标试纸与MPAIA试剂盒检测日本血吸虫病血清抗体应用方向探讨

【摘要】 目的评价SPA金标试纸条和MPAIA试剂盒检测日本血吸虫病血清抗体的应用效果,并探讨其应用方向。方法使用SPA金标试纸和MPAIA试剂盒检测376例日本血吸虫病患者血清、100例健康人血清及120例蠕虫病患者血清,对检测结果进行分析、比较。结果 SPA金标试纸条灵敏度为95.21%(359/376),特异度为97.00%(97/100),MPAIA试剂盒灵敏度为97.61%(367/376),特异度为100%(100/100);两者检测结果差别无统计学意义(日本血吸虫病患者:χ2=2.23,P>0.05;健康人:χ2=0,P>0.05),且呈高度一致(kappa=0.88);SPA金标试纸交叉反应率为7.5%(9/120),MPAIA试剂盒仅为2.5%(3/120)。结论 SPA金标试纸和MPAIA试剂盒均可作为理想的日本血吸虫病诊断试剂;前者操作简便、反应迅速、利于基层及普查使用;后者准确性较高,更适于临床确诊及鉴别诊断。

【Abstract】 To evaluate clinical effect of colloidal gold-SPA strip and MPAIA kit and investigate their related direction,they were applied to detect serum antibody in 120 patients with Schistosoma japonica,100 healthy people,and 120 patients with verminosis.Then,we analysed the results of detection with statistical index as follows: the sensitivity and specificity for colloidal gold-SPA strip were 95.21%(359/376) and 97%(97/100),respectively.The MPAIA kit were 97.61%(367/376) and 100%(100/100),by comparison.There was no difference between the performance of two kits(patients with Schistosoma japonica: χ2=2.23,P>0.05;healthy people: χ2=0,P>0.05),and they were very consistency(kappa=0.88).The cross reaction rate of serum from patients with other verminosis by colloidal gold-SPA strip was 7.5%(9/120),and that of MPAIA kit was 2.5%(3/120).Results indicated that colloidal gold-SPA strip and MPAIA kit are fitting diagnostic reagent to Schistosoma japonica.The former one is rapid and simple enough to be applied to primary hospital and field surveys.However,the latter one is accurate and habituated to make a definite diagnosis and differential diagnosis.
