老年股骨颈骨折四种髋关节置换术式的临床观察

【摘要】 [目的]评价全髋骨水泥及生物置换术,双极骨水泥及生物人工股骨头置换术治疗老年股骨颈骨折的临床分析。[方法]回顾性分析2001年3月~2010年3月期间458例股骨颈骨折患者,分别行全髋骨水泥型关节置换术(107例)、全髋生物型关节置换术(89例)、双极骨水泥型(115例)与双极生物型(147例)人工股骨头置换术治疗。随访时间1.5~9年,平均6.5年。对全部病例的手术时间、手术出血量、引流量、住院时间、有无并发症以及术后功能恢复等情况进行比较。[结果]双极(骨水泥型及生物型)组在手术时间、手术出血量、引流量等方面无明显差异(P>0.05),但均优于全髋骨水泥型及生物型组(P<0.05),4组在术后功能评分和术后疼痛率方面具有明显差异(P<0.05)全髋(骨水泥型及生物型)组优于双极(骨水泥型及生物型)组。[结论]对于相对年轻、身体条件较好的老年患者建议选择全髋(生物型)关节置换术;对于相对年轻的,骨质疏松的老年患者可选择全髋(骨水泥型)关节置换术;对于年龄较大的,或者身体条件较差的相对年轻的老年患者,建议选用双极生物人工股骨头置换;对于年龄较大且骨质疏松明显的患者选用双极骨水泥人工股骨头置换。

【Abstract】 [Objective]To estimate the clinical effects of cement,organism total hip arthroplasty(c-THA or o-THA),bipolar cement or organism hemiar throplasty(c-BHA or o-BHA) for the treatment of femoral neck fractures(FNF) in the elderly. [Method]From March 2001 to March 2010,a follow-up study was done in 458 patients with femoral neck fracture in the elderly. The patients were divided into c-THA(n=107),o-THA(n=89),c-BHA(n=115),o-BHA(n=147) groups.The follow-up duration was from 1.5 to 9 years with an average of 6.5years.The operation time,blood loss,volume of drainage,hospitalization time,complications and functional rehabilitation were compared. [Result]There were no statistical differences in hospitalization time among the four groups.Compared with c-BHA and o-BHA group,the c-THA and o-THA group had a longer operation time and more blood loss,but had a higher functional scores and a lower rate of pain. [Conclusion]o-THA is especially suitable for those who are younger and active,c-THA is especially suitable for those who are younger and with osteoporosis.o-BHA is suited to elderly patients,c-BHA is suited to elderly and osteoporosis patients.
