【摘要】 目的了解下呼吸道真菌感染情况及对抗真菌药物抑菌效果,为临床提供帮助。方法对医院2006年1月-2008年12月临床送检的痰液或纤维支气管镜收集的标本,按照全国临床检验操作规程的有关要求进行分离鉴定,对其中较常见的假丝酵母菌属(106株),采用Rosco纸片扩散法对常用5种抗真菌药物进行药敏试验。结果 2116份痰标本共分离出6种假丝酵母菌属,共422株,检出率19.9%,其中以白色假丝酵母菌为主,占52.1%,其次是热带假丝酵母菌,为20.4%,类星形假丝酵母菌、克柔假丝酵母菌、近平滑假丝酵母菌、高里假丝酵母菌、皱褶假丝酵母菌,分别为12.1%、6.2%、5.0%、2.8%、1.4%;大多为与细菌混合感染;药敏结果为白色假丝酵母菌和类星形假丝酵母菌敏感率大多>90.0%,热带假丝酵母菌敏感率也>80.0%,克柔假丝酵母菌对氟康唑几乎全部耐药。结论下呼吸道假丝酵母菌感染比较严重,且常与细菌合并感染;常见感染假丝酵母菌对常用抗真菌药物有一定耐药性,应引起重视;Rosco纸扩散法操作简单,结果可靠稳定,适用常规抗真菌药敏试验。更多还原

【Abstract】 OBJECTIVE To find out infection and the drug resistance of fungus in the lower respiratory tract for clinical assistant.METHODS Sputum and specimens of branch of bioscope clinical collected from Jan 2006 to Dec 2008 were isolated and identified according to the standard of the guidelines of NCCLS.Rosco card chart method was adopted for antibiotic sensitive test of against 5 species anti-fungus antibiotics in 106 strains of Candida commonly found.RESULTS A total of 422 strains of 6 species Candida were extracted from the 2116 sputum with the detection rate of 19.9%,among which C.albicans accounted for 52.1% of the whole Candida,then the C.tropicalis accounted for 20.4% and C.stellatoidea,C.krusei,C.parapsilosis,C.gulliermondii and C.rugosa which were 12.1%,6.2%,5.0%,2.8%,1.4% respectively.Most of which were acquired mixture infection with other bacteria.The antibiotic sensitivity studies indicated that the sensibility of C.albicans and C.stellatoidea were over 90.0% on the average and sensibility of C.tropicalis was over 80.0% on the average,and C.krusei almost all against the medicine to fluconazole.CONCLUSIONS Infection of Candida is serious in the lower respiratory tract,and is acquired mixture infection with other bacteria.Candida causing commonly seen infections are resistant to anti-fungus drug commonly used with different extent.A great attention should be paid to it.Rosco card chart method is an easy method and is reliable to the result and suitable for routine anti-fungus antibiotic sensitivity test.更多还原
