休克期补液量对小儿大面积烧伤合并脑水肿预后的影响

【摘要】 目的探讨烧伤休克早期不同补液剂量对大面积烧伤合并脑水肿患儿预后的影响。方法回顾性分析新疆维吾尔自治区人民医院烧伤科2005年10月至2010年10月收治的大面积烧伤合并脑水肿患儿35例,按入院前补液量不同而导致脑水肿分为两组,A组20例为补液量不足组,B组15例为补液过量组,对比两组患儿多器官功能障碍综合征(MODS)的发生率及预后。结果 A组发生MODS(90.00%)较B组(33.33%)高,预后较差,两组比较差异有统计学意义(P<0.05)。结论防治小儿大面积烧伤合并脑水肿的关键是早期、准确、有效的液体治疗。在小儿大面积烧伤休克早期,补液的"量"较"质"更加重要。缺血-再灌注损伤是诱发MODS的重要因素。

【Abstract】 Objective In order to discuss the effects of the different fluid volume on the prognosis of children of extensive burn with hydrocephalus children in time of the early burn shock.Methods Thirty-five children cases of the extensive burns with hydrocephalus who had been hospitalized in the department from October 2005 to October 2010 were retrospectively analysed and divided into two groups according to the different fluid volume.Group A(20 cases) was injected the insufficient fluid and group B(15 cases) was injected the excessive fluid.The incidence and prognosis of multiple organ dysfunction syndrome(MODS) were compared in the two groups.Results Group A had a higher incidence of MODS than group B had and a worse prognosis.There was a significant statistics difference between the two groups.Conclusions The key points to prevent and treat children with hydrocephalus in severe burn should be the early,accurate and effective fluid therapy.In the early stage of shock in children’s large area burn,the fluid therapy’s "quantity" is more important than "quality".Induced ischemia-reperfusion injury is an important factor in MODS.
